Job Summary: The Case Manager, Family Service Coordinator, connects families with communityresources, assists with community outreach and engages families in program-drivenservices/activities. The Family Service Coordinator provides individualizedsupport through a family support plan, community resource referrals, potentialhome visits, and ensures successful enrollment in key services; flexible hourswhich includes some nights and weekends is required. This position reports to the Program Director. Specific responsibilities include, but arenot limited to:
Responsibilities/Duties:
- CompleteFamily Resource Scale to assess family needs and help establish goals inidentified service areas
- Assistfamilies in development of a family support plan to complete established goals
- Refer andlink families to appropriate community agencies and ensure necessary follow-up
- Providefamilies with referrals and follow-up via phone and in-person
- Assistfamilies with completion of applications to community services and/or programs
- Make homevisits for families enrolled in service if needed
- Assist indevelopment and implementation of family support programming
- Orient newfamilies to the program services and environment
- Coordinateand facilitate programming and events
- Assist in providing a warm greeting and engagement of neighborhood families into Layla’s house
- Maintaincooperative working relationship with co-workers, supervisors, families,partners and the community
- Maintainconfidentiality of all information at all times
- Research available community resources
- Performtargeted outreach as needed
- Complete accurate and timely data collection to complete reporting requirements.
- Coordinateprogram’s compliance to contract, grant, accreditation and agencyrequirements.
- Attend staff meetings, on-going in-service training and reflective supervision as required.
- Participate on appropriate agency, department and community planning teams.
- Performs other related duties as assigned.
Qualifications/Requirements:
- Minimum Bachelor’s Degree in social work, human services, early childhood or related field is required
- Minimum of 3years’ experience working directly with families
- Ability to be speak fluently and communicate effectively, including written, in English and Spanish is required
- Experiencemaking referrals and linking families to community services
- Experiencein case management preferred
- Ability towork independently
- Ability to work some evenings and weekends
- Professional, self-directed, follow-through on projects and prompt responsiveness to internal and external stakeholders.
- Goodinterpersonal skills, team-oriented, customer service focused enjoys workingwith others
- Strong verbalcommunication skills and demonstrated ability to write clearly and persuasively
- Proficiency in of Microsoft Word, Excel, Outlook, and PowerPoint
- Ability to walk, stand, climb stairs, kneel, bend, reach, and manipulate objects; move materials up to 10 pounds on a regular basis; infrequently require moving materials weighing up to 40 pounds
- Some travel around Hillsborough County required
- Must be sensitive to the culturally diverse population Champions for Children serves
- Must have a reliable car, active auto insurance, and valid Florida driver’s license
- Must be able tosuccessfully complete a criminal background check, motor vehicle records checkand drug screening
