About the Department
The Emergency Medical area of Hennepin Healthcare's Downtown Minneapolis Hospital is seeking to hire two Senior Social Workers to join the team. In these positions, you will provide behavioral health and crisis supports to individuals who are brought to the hospital. You will facilitate immediate service coordination with community providers, connect residents to new and existing supports, and participate in resident discharge planning and coordination of services for residents who are identified as high risk for re-admission.

Location and hours:
These positions are located at the Hennepin Healthcare Emergency Department at 701 S 8th Street, Minneapolis, MN 55404. Work hours will be between 3 p.m. - 11:30 p.m. You will work 5 shifts a week, weekends will be required. Specific schedule will be discussed further with hiring team. New Employee Orientation (NEO) for all new benefit-earning employees is in-person in downtown Minneapolis. Details about NEO will be provided upon hire.

In these positions, you will:
- Conduct in-person screenings, care coordination, facilitation of crisis assessments, de-escalation, stabilization and clinical services, and documentation in design systems.
- Consult with non-licensed practitioners and review practitioner documentation.
- Interview clients and perform case evaluations to determine problems and appropriate types and methods of service/treatment.
- Design and follow through on short-term service plans for residents as they stay in the emergency department.
- Work with the unhoused community providing behavioral health and mental health services.
- Work as part of a multi-disciplinary team, providing work directions to the emergency room social workers to ensure best patientcare practices.
- Provide leadership with collaboration and continuing education.
- Provide supervision, training, and facilitation of unit meetings.
- One of the following:
- Master's degree or higher in social work or closely related field with course work emphasis in areas such as social work methods, human growth and behavior, and principles of social welfare.
- Bachelor's degree or higher in social work, psychology, sociology, or human services, and 18 semester (27 quarter) credits of graduate work in the above areas, and three years or more of full-time supervised experience as a social worker.
- Licensed as a Graduate Social Worker by the Minnesota Board of Social Work.
- A valid driver's license and the ability to obtain a Hennepin County driver's permit.
- Experience:
- Providing behavioral health, mental health, and crisis supports to individuals who are brought into a hospital.
- Working with the un-housed community.
- Facilitating immediate service coordination with community partners.
- Connecting residents to new and existing supports.
- Consulting with non-licensed practitioners and reviewing practitioner documentation.
- Ability to:
- Work well independently and as a team.
- Demonstrate strong interpersonal, organizational, and computer skills.
- Communicate both verbally and in writing in Hmong, Spanish, or Somali.
- Knowledge of short-term service plans for residents.
