DAY BREAK ACTIVITY AIDE
Job Summary: The CNA/Activity Aid will provide immediate personal care to clients; observe and report any changes in physical condition or behavior to program director and RN consultant, assist with planning, preparing and implementing daily activity program and serve meals/snacks and assist with clean-up as assigned.
Job Relationships:
A. Reports to: Adult Day Manager or Assistant in absence of Manager
B. Interrelationships with: Program assistant, Activity Aides, Volunteers, RN,
C. Supervisory responsibilities (or internal contacts). No supervisory responsibilities
D. External Contacts: Client’s families, Bus/van drivers,
Hours of Work: Hours as scheduled 2-3 Day a week
Dress Code: Street clothing appropriate in a business environment with identification name badge.
Essential Requirements of Work: (Minimum qualifications necessary to function at full productivity)
A. Education and Training: High school diploma or equivalent
NA/R training and certification (not required)
CPR and First Aid training and certification
B. Experience:
Previous work with elderly or physically challenged persons is desirable.

Physical Demands of the Job:
1. Frequent walking, twisting, standing, bending/stooping, pushing/pulling movements

Principle Job Responsibilities:
Responsibility: Assisting with client care and programming.
Tasks:
1. Clients are treated with care and respect. They are assisted with transfers and toileting needs as necessary, in a timely manner. Other client requests are handled promptly.
2. Bathe assigned clients, clean w/p or tub and chair after each use.
3. Observe and report any incidents, changes in condition or behavior to Program Director/RN Consultant.
4. Assist clients with bowel and bladder cares and maintain cleanliness
5. Provide aseptic techniques to prevent spread of disease, infection
6. Take TPR’s, BP’s and weights as assigned.
7. Assist with meal serving and snack preparation in accordance with food handling policies, as assigned.
8. Answer phone.
9. Assist with charting of client monthly progress notes.
10. Assist with clean-up of program area as assigned.
11. Share pertinent client information with Program Director and team members.
Responsibility: Make daily reminder calls to clients and establish attendance list
Tasks:
1. Check with program assistant before making calls for pertinent client information for the day.
2. Call clients
3. Establish list of clients attending for the day.
4. Check for client appointments outside of ADS or other changes in clients’ schedules for the day.
5. Coordinate client daily schedule and attendance list with passenger assist’s schedule and alert program assistant to changes in client schedules.
6. Relate pertinent information to Program Director/staff regarding client’s schedules for the day.
Responsibility: Provide leadership for planning monthly client activity calendars.
Tasks:
1. Collect appropriate activity data
2. Present data to staff for discussion.
3. Work with assigned staff to insure completion of activity calendar making arrangements for speakers and entertainment.
4. Work with assigned staff to insure all needed supplies are present for activity programming.
5. Be involved with monthly assessment of client activities.
