Care Manager – Home Care by Design

What you will do:
As a Care Manager, you will be the vital link between clients, families, and caregivers. You will build lasting relationships by:
· Understanding and tailoring home care solutions to each client’s unique needs.
· Coordinating caregivers with thoughtfulness and attention to detail.
· Communicating and documenting important caregiver and client feedback.
· Complete quarterly surveys to assess client satisfaction.
· Supporting our team to deliver care that not only meets but exceeds expectations.

Requirements
What we’re looking for: We want someone who is:

- A background in customer service, preferably in a health care setting.

- Experience in home care, hospice, and other client facing roles in health care preferred.
- Skilled at quickly developing relationships and loyalty with a client-service oriented approach
- Computer skills: Microsoft office, comfortable in learning client care management software systems
- Must be available on weekends as clients' needs come up
