🎯 What You’ll Do in This Role:
As a Chaplain, you will provide compassionate spiritual care and emotional support to participants, their families, and caregivers. You’ll lead meaningful spiritual services, offer guidance during life’s most challenging moments, and coordinate support for End-of-Life (EOL) care programs. This role is vital in ensuring that each individual feels respected, supported, and comforted throughout their journey.
Primary Responsibilities:
- Provide spiritual care to participants receiving comfort care and support for those at End-of-Life (EOL).
- Offer Bible studies, Communion, and devotional time as scheduled or upon request in collaboration with the Recreation Therapy department.
- Counsel participants seeking spiritual guidance, offering compassionate support tailored to individual needs.
- Conduct spiritual assessments, develop personalized spiritual care plans, and document services in the electronic health record.
- Lead and facilitate various spiritual programs, including Spiritual Wellness sessions, memorial services, aging gracefully sessions, grief support groups, and Bible studies.
- Conduct bereavement assessments for caregivers and families, providing grief support through condolence calls, support groups, mailings, and memorial services.
- Collaborate with local faith communities and leaders to foster partnerships and enhance available spiritual resources.
- Provide training and development to team members on topics related to spiritual care.
- Assist with oversight and monitoring of EOL program goals, including quality improvement planning.
- Communicate effectively with the interdisciplinary team, participants, caregivers, and families.
- Uphold all Thome PACE standards, policies, and procedures while promoting a full continuum of spiritual care services.
✅ What You’ll Need:
- Minimum of one (1) year of experience working with frail and elderly populations.
- At least one (1) year of chaplain and/or pastoral experience.
- Bachelor’s degree in Theology, Counseling, Psychology, Social Work, or Human Services required.
- Strong problem-solving skills with the ability to work independently and adapt to a fast-paced, ever-changing environment.
- Proficiency in using electronic systems and applications for charting, record-keeping, and participant communication.
- Reliable transportation with valid driver’s license and current auto insurance; ability to travel to off-site locations as needed.
