Job Title: Shelter Case Manager 

Location/Work Status: Windward Oahu, Hawaii 

Job Type: Regular Full Time, Non-Exempt

Summary of Position: 

Parents And Children Together operates confidential domestic violence shelters which offer healing environments for families and individuals fleeing dangerous situations. Services include safety planning, case management and referrals to legal, medical, financial and housing assistance.

The primary responsibilities of the Shelter Case Manager are to provide advocacy and case management support services to domestic violence survivors who are admitted to the Shelter. Services include individual advocacy, crisis intervention, and individual counseling for clients admitted to the shelter including intervention and crisis counseling for clients experiencing episodes of trauma response, mental health crisis, substance use crisis, or medical emergencies.  This position also manages the intake and discharge procedures for each client as assigned.  This position is under the direct supervision of the Shelter Manager.

Essential Functions:

1. Behaves in a manner that supports the agency's mission and follows personnel policies and procedures.

2. Provides case management services through weekly meetings with assigned clients-including life skills building, domestic violence education, increasing knowledge and skills of community resources, individualized safety planning, assistance with filing a temporary restraining order, legal services advocacy, development of healthy coping skills, management of mental health, substance use intervention, and conflict resolution.

3. Manages assigned caseloads in addition to the completion of required client intake and discharge processes, including comprehensive assessments, determination of safety risks of the client, and provides each client with thorough walk-throughs and orientation into the shelter.

4. Provides case management, individual advocacy, crisis intervention, and individual counseling for clients admitted to the shelter and notifies the Shelter Manager in the event of crisis situations.

5. Provides direct services to survivors in PACT's Rapid Re-Housing program. Provides counseling directed to transition planning, financial literacy planning, attainment of housing, and assistance or referrals relating to other factors that influence the ability for survivors to succeed on their own. Works directly with community resources such as landlords, housing authorities and others to promote and coordinate victim transition services. Coordinates rent collection when needed and works with agency management when rent payment is delinquent.

6. Provides crisis intervention and crisis counseling for clients experiencing episodes of trauma response, mental health crisis, or substance use crisis.

7. Provides clear, concise, and non-judgmental written documentation such as assessments, service planning, progress notes, opening and closing summaries, DHS reporting, follow-up calls, monitoring, collateral contacts, crisis calls and other pertinent information in client individual files in a timely manner. Is responsible for maintenance of case records and for accurate data input of client information into eHana and any other required database systems.

8. Confers with the Shelter Manager for the implementation of general changes to the intake, assessment, group facilitation processes, procedures, training, or programmatic activities.

 Education / Training: 

 1. Bachelor’s degree in social work, psychology, counseling, or equivalent human services field, required PLUS one (1) year experience providing case management services in the field of intimate partner violence plus one (1) year experience working with children in a group or individual setting, preferred, OR 

2. High School Diploma, required PLUS three (3) years’ experience providing case management services in the field of intimate partner violence plus three (3) years’ experience working with children in a group or individual setting, required

Other: 

  1. Possesses and maintains a valid driver’s license. 
  2. Has daily access to an automobile.
  3. Possesses and maintains proof of no-fault insurance.
  4. Can obtain an acceptable pre-hire driver’s abstract and annually thereafter. 
  5. Can obtain an acceptable Fingerprint (FBI) Background Check upon initial hire and as requested. 
  6. Can obtain a State Criminal History check upon initial hire, one year later, and every other year thereafter or as requested. 
  7. Can obtain an acceptable CWS child abuse registry check upon initial hire, one year later, and every other year thereafter or as requested. 
  8. Can obtain an acceptable APS adult protective registry check upon initial hire, one year later, and every other year thereafter or as requested. 
  9. Can obtain an acceptable National Sex Offender child abuse registry check upon initial hire, one year later, and every other year thereafter or as requested.
  10. Can complete an initial tuberculosis test prior to hiring or have documentation of a negative test result within one year of the date of hire, and annually thereafter.
  11. Must have a cell phone for use in emergencies at off-site locations.
