Position Summary:
The Case Manager is the member of the care team responsible for initiating and maintaining effective working relationships with youth, families, placement agencies, and community resources--prior to admission, through placement, and after discharge. The Case Manager works with the youth, family and/or placing agency to evaluate the youth’s progress toward goal attainment. The Case Manager works to promote and identify resources available for the youth and family in their home community to build natural supports that will aid in their transition from Everstand. An essential responsibility of Case Management is family engagement and identification of strengths and needs.
In PA, the Case Manager acts in a dual role, supporting both the case management and planning of therapeutic services for all youth. At this site, the Case Manager works closely with all members of the care team to design and implement guidance and treatment plans for the youth in order toto develop, support and encourage safe behaviors, self-regulation and healthy relationships.
Essential Functions:

· Complete admission service needs assessments that include participation by child/youth, parents/guardians in the identification of strengths, service needs, and discharge planning.
· Participate in regular internal and external meetings, court appointments, school meetings, community providers meetings, group, individual and family meetings as scheduled.
· Participate as an active member of a care team for each youth on an assigned caseload. Understanding that the care team includes members who are employed at Everstand as well as the youth’s family, placing agency, church community, and any support that the youth and family identify.
· Take the lead role on a rotating basis on planning and implementing a Family Engagement Event for the youth and their identified supports.
· Promote an understanding of available Everstand and community resources.
· Locate and facilitate referrals to community-based resources within the youth’s home community. Coordinate with Everstand staff members and community resources to address clients’ service needs. Ensure that the service needs are met.
· Evaluate the child’s/youth’s/family’s progress towards goal attainment, in line with scheduled treatment planning meetings.
· Maintain effective working relationships with families, children/youth, colleagues, consultants and community agencies to include scheduling home visits, notifications of serious incidents, and follow-up on all ongoing documentation needs.
· Ensure post-discharge follow-up—30 days, 60 days, and 90 days after discharge—to collect data that accurately reflects the residual impact of treatment.
· Complete required documentation and reports within the defined timelines including case management notes, state data collection, assessments, serious incident reports, treatment plan documentation, discharge planning, post discharge follow-up.
· Provide effective case management to assigned cases which include but is not limited to: liaison with internal and external professionals such as health professionals, schools, placing agencies, lawyers, community partners, and other human service agencies.
· Ensure assigned case records meet documentation standards including monthly chart audits. Submit by the 15th of each month to the Admin Asst. documentation of the chart audit.
· Identify supervision and/or professional training needs to supervisor as they arise.
· Demonstrate the ability to effectively express ideas clearly and concisely, orally and in writing.
· Demonstrate the ability to engage children and families in a positive therapeutic alliance
· Facilitate life skills/social skills groups on a weekly basis, ensuring that there is proper documentation for these groups.

Work Environment:
This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
Being at a residential and school living environment there may be times you experience conditions in which challenging behavior can be exhibited from our youth to include but not limited to, physical agitation, physical violence and/or emotion outburst. This position will require that you enter the homes and communities of the youth and families that we serve, you will need to have knowledge of safe home visiting practices.

Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception and the ability to adjust focus. Ability to complete Therapeutic Crisis Intervention training and be able to physically participate in a restraint if needed.
Position Type/Expected Hours of Work:
This is a full-time position. Days and hours of work are typically Monday through Friday, 8:30 a.m. to 5 p.m. This position does require that Case Manager work one late night each week. This late night can be either 11a-7p or 12p-8p. This position regularly requires long hours and weekend work. Eligible for flex schedule.
Travel
Travel is primarily local during the business day, although some out-of-area and overnight travel may be expected. Also, transportation of clients to community based meetings to include court hearings, child in placement review meetings will be expected.
Required Education and Experience
Bachelor’s Degree with at least one-year of experience in providing direct services to children/youth and families
Preferred Education and Experience
Master’s Degree in Social Work, Counseling, or a related field from an accredited college or University
Current LSW, LPC, LCSW, LPC, LMFT, LPAT, or LSW/LCPAT in Pennsylvania State License
Additional Eligibility Qualifications
· Must be able to type 50 wpm.
· Able to learn and effectively utilize different technology platforms.
· Excellent computer use knowledge with Microsoft Windows 10 and 365 Office products, including Word, Excel, PowerPoint, and Outlook.
· Proven effective time management, planning, communication, and interpersonal skills
· Ability to problem solve.
· Ability to work independently.
· Ability to keep an electronic calendar (Outlook).
· Ability to pass RCYCP certification both at employee orientation and per the schedule outlined by HR thereafter.
· Ability to pass TCI aid certification both at employee orientation and per the schedule outlined by HR thereafter.
· Ability to pass CPR/First aid certification both at employee orientation and per the schedule outlined by HR thereafter.
· Ability to demonstrate the possession of a valid driver’s license prior to employment and ability to maintain the license and operate a motor vehicle thereafter.
· Ability to pass CPS/UII background check both prior to employment and per the schedule outlined by HR thereafter.
· Ability to pass a Tuberculosis Screening both prior to employment and per the schedule outlined by HR thereafter.
· Ability to pass a work-readiness physical performed by a certified medical professional both prior to employment and per the schedule outlined by HR thereafter.
· Ability to pass a Federal and State Fingerprinting Background Check both prior to employment and per the schedule outlined by HR thereafter.
· Ability to pass a drug screen via urinalysis both prior to employment and randomly thereafter.
· Client file maintenance.
