Summary: The Intake Coordinator is primarily responsible for collecting and keeping demographic information on clients and routing this information to the appropriate personnel. The intake coordinator will assess the clinical needs and urgency of consumers requesting behavioral health/clinical services in accordance with established professional and ethical guidelines and established policies and procedures. 

 

Interrelationship: Will interact and communicate with Clinical staff and support staff as it relates to required services. 

Primary Job Responsibilities: 

  • Provide telephone and in-person triage, crisis evaluation, and crisis management for consumer to assist in prioritization, scheduling and assignment of new clients.
  • Conduct intake interviews, including administration and interpretation of a psychological inventory, client questionnaire, and medical/psychological history, 
  • Assign consumers to clinicians or group therapy and prepare intake summary and write-ups using the electronic records system.
  • Coordinate consumers psychological services, including consultation with psychiatrist, referral to appropriate community resources, follow-up, and case disposition.
  • Maintain clinical records in accordance with agency policies and state/federal regulations.
  • Participate in scheduled meetings, including staff meetings and clinical meetings.
  • Participate in trainings to maintain and enhance job skills.
  • Applies legal precedents that relate to ethical conduct and consumers right.
  • Perform, as assigned, other administrative responsibilities associated with counseling and consultative services including, but not limited to participation in committees and Implementation of research activities and special projects.

Job Skills: 

  • Must have excellent verbal communication skills, active listening skills and exceptional customer service skills to evaluate patients, assess their care needs and guide through the admissions process.
  • Proficient with data entry and other technical computer skills requiring information to be entered into digital systems, accessing digital forms, and creating electronic patient files.
  • Must have excellent time management skills to schedule appointments for patients and manage multiple patients at once.

Education/Experience: 

  • High School Diploma or Equivalence, Bachelor’s Degree in behavioral science, psychology, counseling, social work, or a related field of study
  • Experience working in a clinical setting performing clinical related functions is preferred.
