🕒 Schedule: Exempt, 40 min hours per week | Monday - Friday | 8am - 4:30pm

🏢 Department: Outpatient Clinic & Community | LifeCircles PACE | Holland, MI
🎯 What You’ll Do in This Role:
LifeCircles PACE delivers primary care, palliative care, and end-of-life care for participants 55 and older. The RN Clinical Coordinator primary work setting is an outpatient clinic with care also provided in participants homes and facilities. This position is perfect for the RN who wants to be patient-centered, creative, autonomous, and collaborative in their approach to healthcare.
Primary Responsibilities:
- Work closely with other RNs, APPs, and MAs in a well-supported, team-based environment
- Spend 60-70% of your time in the clinic and 30-40% in the community
- Participate in regular interdisciplinary team meetings
- Complete RN health assessments and participate in team-based care planning
- Provide care in various settings (outpatient clinic, patient’s homes, Assisted Living Facilities, and Skilled Nursing Facilities)
- Wound care and other skilled nursing tasks
- Assess acute needs and illnesses
- Patient and caregiver education
- Case management and clinical coordination
✅ What You’ll Need:
- RN-Nursing Diploma or Degree from accredited School of Nursing or University
- Current license to practice as a Registered Nurse (RN) in the state of Michigan
- Possess current CPR certification by AHA or willing and able to obtain as offered by LifeCircles
- Minimum of one year of experience working with the frail elderly or long-term care population.
- Previous experience in home care or long-term care is preferred.
