The opportunity

 

We have exciting opportunities for experienced Home Care Package Coordinators, Enrolled Nurses or CHSP Coordinators seeking to move into a Care Partner role and join our Aged Community Care Team on a permanent, fulltime basis at Park Holme or Elizabeth South. This position offers the flexibility of a hybrid work arrangement, allowing you to work from home and our offices.

 

You’ll work closely with clients, carers, and families in the community to deliver person-centred care that supports independence and wellbeing.

Key Responsibilities

 

  • Provide person-centred case management and coordinate services to support clients and carers, including complex, high and moderate care

  • Conduct strengths-based assessments and develop tailored care plans aligned with client needs and contractual obligations

  • Manage HCP budgets and ensure resources are used effectively

  • Liaise with health professionals, community organisations, and internal teams to ensure holistic and coordinated client care delivery

  • Lead and support direct care staff

  • Ensure safe work practices through workplace assessments, hazard identification, and follow OSH guidelines

  • Maintain accurate records and meet reporting requirements

  • Navigate challenges such as limited resources, evolving technology, and complex client behaviours with resilience and adaptability

About you

 

You will have a strong background within the Aged Care Industry and have recent demonstrated HCP experience and/or strong knowledge, in addition you will be able to demonstrate the following experience, skills and attributes:

 

 

  • Certificate IV in Community Services (or equivalent) and a current First Aid
  • Strong experience in case management, care planning, and HCP budgets
  • Knowledge of service provision, coordination, allocation and compliance within a community environment
  • Strong decision-making, problem-solving, and time management skills
  • Able to work independently and as part of a team
  • Excellent communication and interpersonal skills
  • Confident using computer systems (Word, Excel, calendars)
