Treatment Coordinator - Behavioral Health (West Valley) presso Jewish Family & Children's Service

*West Valley Location**

Treatment Coordinator: The Treatment Coordinator provides ongoing care coordination services to assist in addressing the client and family’s ongoing service needs, including both primary care and mental/behavioral health. This position assists clients and families in obtaining community support, identifying resources, and resolving barriers.

• Performs ongoing coordination of services to children and adults in accordance with the treatment plan.
• Responds to initial crisis calls and identifies needs & necessary resources accordingly.
• Uses established best practices to engage formal and informal resources for the client, including the transition, discharge, and aftercare plans of behavioral-health services while demonstrating a positive, strengths-based attitude of teamwork and shared leadership.
• Provides coordination of behavioral-health services with other agencies, such as Department of Child Safety (DCS), Probation/Parole Departments, Division of Developmental Disabilities (DDD), Arizona Long Term Care, schools, and other providers.

• Bachelor's Degree in human services field; a combination of experience and education may be considered
• Relevant experience working in a behavioral health setting
• Level 1 Fingerprint Clearance Card
