Home Healthcare, Hospice and Community Services is dedicated to supporting families in building safe, healthy, and nurturing environments where children and parents can thrive.
The Family Support Specialist plays a vital role in connecting parents and pregnant families with essential resources and services to strengthen family well-being. Through regular home visits and individualized support, the specialist builds trusting relationships, provides guidance, and links families to community resources that meet their unique needs — from healthcare and housing to education, employment, and parenting support.
Key Responsibilities
Conduct regular home visits with pregnant individuals and parenting families to provide support, encouragement, and resource navigation.
Build and maintain trusting, respectful relationships that promote family strengths and self-sufficiency.
Assess family needs and collaboratively develop service plans to address areas such as healthcare access, nutrition, housing, early childhood development, parenting skills, and financial stability.
Connect families with appropriate community resources, programs, and services, and assist with referrals and applications as needed.
Provide basic parenting education, child development information, and support for healthy family relationships.
Collaborate closely with community partners and service providers to coordinate care and ensure comprehensive support.
Maintain accurate, timely documentation of home visits, family progress, and referrals in accordance with program requirements.
Participate in team meetings, ongoing training, and program evaluation activities.
Requirements
Successful completion of high school or GED high school equivalency diploma;
Successful parenting experience; experience in negotiating community services and accessing services as needed;
Knowledge of stages of pregnancy, normal child growth and development and age appropriate child nutrition and play;
Ability to follow care plan as determined by the Healthy Starts Team and within the boundaries set by the plan;
Valid driver’s license and access to an automobile during working hours that meet the State of New Hampshire registration regulations and auto liability insurance.
