The Role: Care Navigator
The Care Navigator is the heart of our concierge services program — a trusted guide who blends clinical expertise, care coordination, and world-class hospitality to create seamless transitions for residents and families. Whether it’s moving into Independent Living, transitioning to Assisted Living or Memory Care, or returning home after a hospital stay, the Care Navigator ensures every step of the journey is thoughtfully planned and emotionally supported.
This is more than a clinical role. It’s about curating experiences that are uplifting, worry-free, and reflective of each resident’s unique story.
What You’ll Do
- Resident & Family Support: Be the first point of contact, offering reassurance, warmth, and clear communication.
- Care Coordination: Curate and coordinate personalized transition experiences, from move-ins to hospital discharges.
- Insurance Guidance: Simplify Medicare, insurance, and billing processes for residents and families with clarity and confidence.
- Ongoing Connection: Maintain relationships during hospital stays, rehab, or respite care, ensuring continuity and peace of mind.
- Hospitality & Leadership: Anticipate needs, reduce stress, and collaborate with colleagues to elevate the resident and family experience.
What We’re Looking For
- Licensed Nurse in the State of Michigan.
- At least 3 years of experience in health care or senior living setting managing transitions across multiple levels of care.
- Knowledge of insurance coverage and authorization processes preferred.
- Excellent communication, organizational, and interpersonal skills.
- Proficiency with electronic medical records, spreadsheets, and care coordination tools.
