We’re looking for a dedicated Physical Therapist to provide high-quality care across two senior living communities in South Bend and New Carlisle, Indiana. This role is an opportunity to make a meaningful difference every day, helping residents maintain independence and quality of life. You’ll be part of a therapist-owned company that prioritizes collaboration, support, and clinical growth.

Position Highlights

· Full-time schedule, Monday–Friday (30–40 hours per week)

· Paid travel time and mileage between communities

· Close-knit, interdisciplinary teams that value communication and cooperation

Community Details

South Bend:

· Located on the city’s south side, offering a full continuum of care in a lively, team-oriented setting

· Services include short-term rehab, long-term care, outpatient, and assisted living

· Modern therapy gym with an ADL kitchen and bathroom for real-life practice.

New Carlisle:

· Centrally located with a welcoming, home-like campus and landscaped grounds

· Levels of care include assisted living, skilled nursing, and continued rehab services

· Three therapy spaces: a dedicated PT gym, OT gym, and independent workout area, plus a training kitchen and bathroom.

What You Bring:

• Active Indiana Physical Therapy license (or eligible to obtain)
• Graduate of a CAPTE-accredited Physical Therapy program
• Passion for working with older adults and improving quality of life
• Strong communication and collaboration skills

Requirements

Applicants must have an active Indiana Physical Therapist license or must have the necessary qualifications to earn Indiana license. These qualifications include earning an advanced degree from a Commission on Accreditation in Physical Therapy Education-accredited physical therapist education program, passing the state licensure exam, and fulfilling the state’s requirements for licensure.

Applicants must be legally eligible to work in the United States. Proof of eligibility will be required upon offer of employment.
