Abode, one of the largest and effective nonprofits working to end homelessness in the Bay Area, is seeking a Service Liaison for our programs in Alameda County.

About The Role: The Service Liaison is responsible for ensuring that the participants placed into permanent housing by the Abode Alameda County Housing teams are connected to and receiving services from their service provider. When necessary, the Service Liaison will step in to address pressing housing stability issues and manage housing crises if the service provider is not immediately available.

How You Make An Impact:  

  • Provide short-term support to tenants in the RALE housing programs when they are experiencing issues that impact their housing stability. 
  • Support tenants with housing retention plan completion. Provide in person follow up and monitoring periodically after relocation to ensure success.  
  • Provide short-term support to tenants who have graduated from services or are disengaged from services when there is a new issue impacting their housing stability. 
  • Support with linking tenants back to their Housing Community Support (HCS) provider, 
  • Assess the need for HCS services and complete HCS referral form. Ensure county assigns a new HCS provider and support with linking tenant with their new HCS provider.  
  • Attend team meetings and other housing conferencing meetings weekly as determined by your supervisor. 
  • Provide outreach to households that are pre-crisis and may need support in the future. Provide outreach to households have histories of struggling in housing and/or sites where landlords have requested additional support on site. 
  • Report tenant and unit observations to Housing Specialist and make recommendation for referral to Service Liaison be made by Housing Specialist. Housing Specialist will determine if referral is warranted and make referral as needed. Referral will be reviewed and approved by the Housing Manager. 
  • Participate in partner coordination/case conferencing meetings to discuss housing challenges and ensure team responsiveness. 
  • Document case notes in WMAT. 
  • Share successes with HCS providers and housing team. Work with development team to provide success stories highlighting the success of the RALE. 
  • Communicate updates consistently to appropriate care team members. 
  • Support overall RALE team as needed with special projects, preparation for audits/monitoring, administrative tasks, etc. as deemed appropriate and necessary by Program Manager. 
  • Maintain file compliance and participate in regular file audits.  
  • Other duties as assigned. 

How You Meet Qualifications:  

  • BA/BS required. MS/MSW preferred  
  • A minimum of two years of experience working with the chronically homeless population is required. 
  • People with homeless lived in experience are highly encouraged to apply. 
  • Bilingual applicants highly encouraged to apply especially those fluent in Spanish. 
  • Must have strong crisis intervention and engagement skills. 
  • Knowledge of federal, state, and local fair housing laws. 
  • Strong organizational, detail oriented, and time management skills.  
  • Attentiveness to detail. 
  • Highly motivated, self-directed, flexible, reliable, and collaborative. 
  • Excellent written and oral communication skills. 
  • Reliable transportation along with valid and current CA Driver’s license and insurance is required.
