Position Summary:

Delta Health is seeking a compassionate and dedicated Certified Nurses Aide to join our healthcare team. Under the direct supervision of a licensed RN, the CNA will deliver high-quality, hands-on patient care to individuals ranging from newborn to geriatric, including those in the Transitions-Post Acute Care (T-PAC) program. This role is critical in promoting patient comfort, safety, and satisfaction in accordance with hospital policies and scope of practice.


Key Responsibilities:

  • Provide direct patient care under supervision of a licensed nurse in accordance with hospital policies and procedures.
  • Participate in shift change nursing reports; communicate effectively regarding patient status.
  • Assist nursing staff with patient hygiene, mobility, and other physical care tasks.
  • Monitor patients and accurately document observations and care provided.
  • Recognize and report changes in patient condition to the supervising nurse.
  • Ensure patient and family needs are identified and met with compassion and professionalism.
  • Conduct regular rounds and maintain proper documentation.
  • Assist with or independently transport patients as required.
  • Maintain a clean, organized, and well-stocked work area.
  • Prioritize patient care tasks to ensure timely and efficient care.
  • Document patient care activities in the Electronic Health Record (EHR) system.

Qualifications:

  • Education: Completion of a state-approved CNA training program.
  • Certification:
    • CNA certification required or must be obtained within 60 days of hire.
    • Current enrollment in an RN or LPN program may be accepted in lieu of certification (transcripts required).
    • Basic Life Support (BLS) certification required or must be obtained within 60 days of hire.
  • Experience:
    • At least one year of patient care experience preferred.
