Communicable Disease Investigator and Health Educator presso Visiting Nurse Association

1. Performs all tasks related to case investigation for the VNA contracted Towns via MAVEN

2. Participates in all local and state level meetings/trainings to stay apprised of updated guidance

3. Communicates the status of disease patterns and trends to local community partners

4. Develops and presents educational presentations on various health topics upon request

5. Leads community exercise programs upon request

6. Supports the Department with administrative duties as needed

7. Consistently provides service excellence to all patients, family members, visitors, volunteers and co-workers.

• Bachelor’s degree recommended, Public Health or related major is ideal
• Excellent oral and written communications skills a must
• Experience in customer service a plus
• Must be a critical thinker and be able to handle confidential information with discretion
• Excellent computer knowledge
