This job provides direct spiritual care services to Centra’s patients, their families and its staff.  The Chaplain assesses the holistic needs of patients, families and staff ensuring these needs are addressed in a timely manner providing appropriate crisis and bereavement ministry.  Will also support the needs of Centra’s Clinical Pastoral Education program.

Responsibilities

Conducts spiritual assessments on patients and families to offer patient/family centered care.

Responds to crisis needs providing necessary support upon arrival.

Assists families and staff at time of patient death meeting spiritual, emotional and practical needs including bereavement needs and supporting the completion of the body release form.

Supports staff in times of crisis offering appropriate one-on-one support, code lavender support, leading a debriefing, and/or referring to the Employee Assistance Program when needed.

Serves as liaison for patients and family connecting them to the appropriate faith community.

Charting of ministry in the electronic medical record to support holistic, interdisciplinary care.

Supports the advance care planning function within Centra educating individuals on the advance directive, offering to help complete when appropriate.

Facilitates group work within the system including spirituality groups, bereavement groups, staff support groups, and other support groups as needed.

Acts as the liaison for all Ethics Consults working closely with Centra’s Ethicist to support an ethical recommendation.

Provides mentoring, education and general support for the Clinical Pastoral Education (CPE) program.

Facilitates support for Centra and community needs offering prayers at events, educational opportunities, blessing of the hands, and other requests as needed.

Participates in 2nd on Call function supporting the 24/7 on call commitment of Spiritual Care and Education.

Advocates for the needs of patients by reaching out to appropriate leadership.

Performs other duties as assigned.

 

Additional Requirements for Hospice:

Attends interdisciplinary team meetings to review and update the plan of care for spiritual services.

Serves as a liaison and support to community Chaplains and spiritual counselors.

Provides funeral or memorial services for patients as requested.

Collaborates with community clergy or spiritual counselor to assist in spiritual care of patients and families upon request.

Networks with faith communities for the purpose of educating on hospice philosophy.

 

Other Functions:

Attends Interdisciplinary Team Meetings (IDT) supporting holistic needs.

Participates on organizational committees representing Spiritual Care. 

Represents the Helping Hands Resource to staff.

Qualifications

Required Qualifications:

Master’s degree in Psychology, Theology, or Philosophy

4 units of Clinical Pastoral Education offered through ACPE

Eligible for board certification with Association of Professional Chaplains, National Association of Veterans Affairs Chaplains, National Association of Jewish Chaplains, and/or National Association of Catholic Chaplains

1 year of ministry experience

Preferred Qualifications:

Master’s degree in Seminary 

1 year of general ministry experience in the hospital, hospice, or long-term care setting

Board Certified Chaplain through Association of Professional Chaplains, National Association of Veterans Affairs Chaplains, National Association of Jewish Chaplains, and/or National Association of Catholic Chaplains
