Position Summary: The Intake Supervisor will lead and manage the intake team, ensuring effective training, scheduling, and performance. This role oversees the entire referral process, ensuring timely review, documentation, and assignment of cases. You'll be the key liaison for patients, families, and healthcare providers, addressing concerns and facilitating a smooth transition into care. The Intake Supervisor also plays a critical role in the design, training, and implementation of new software, technology, and standard operating procedures (SOPs) to enhance efficiency, compliance, and overall team performance.

Essential Duties and Responsibilities


  • Supervise Intake Team: Manage and lead the intake team, including interviews and selections, training, scheduling, ensuring productivity, and collaborating with the Director on performance evaluations. Ensure that all team members adhere to HealthFlex policies and procedures.
  • Referral Management: Oversee the processing of all incoming referrals, ensuring that they are reviewed, documented, and assigned to the appropriate staff in a timely manner.
  • Compliance and Documentation: Ensure that all intake procedures comply with federal, state, and local regulations, as well as company policies. Maintain accurate and complete documentation for all referrals and patient information.
  • Technology & SOP Implementation: Lead the design, rollout, and training of new software, systems, and technology tools to support intake operations. Develop, implement, and update SOPs to standardize processes, enhance efficiency, and ensure compliance.
  • Quality Assurance: Monitor the quality of the intake process and implement improvements as necessary. Conduct regular audits of intake documentation and procedures.
  • Collaboration: Work closely with clinical and administrative teams to ensure coordinated and seamless patient care. Participate in interdisciplinary meetings and contribute to care planning.
    Reporting: Prepare and present regular reports on intake activities, referral trends, and team performance to senior management.
  • Quality Assurance: Monitor the quality of the intake process and implement improvements as necessary. Conduct regular audits of intake documentation and procedures. Proactively identify barriers to timely services and admissions, collaborate with internal teams to resolve challenges, and implement solutions that streamline the intake process and improve patient access to care.

Schedule: Monday-Friday, 9 am to 5 pm, with 1 day a week remote.

Requirements

  • Minimum of 1 year of intake experience in a home health setting required
  • Prior leadership experience is highly desired
  • Knowledge of Home Health regulations 
  • Proficiency in electronic health records (EHR) systems and Google Office Suite
