The RN Navigator- Infectious Disease will collaborate with the infectious disease patient population and the care team to coordinate care that ensures the delivery of quality, efficient and cost-effective healthcare services. Under provider direction, they give patient education related to their illness and monitor planned treatment. The RN will work in collaboration with the entire healthcare team to manage transition of care between the in-patient and office setting to optimize patient outcomes.

Principal Responsibilities
- Assess, plan, implement, coordinate, monitor and evaluate all options and services with the goal of optimizing the specific population’s health status.
- Collaborate with patient care teams to enhance evidence-based clinical guideline adherence and promote best practice by initiating/adjusting therapies as directed by the practitioner and providing appropriate follow-up and monitoring as needed.
- Coordinate and manages appropriate treatment regimen as directed by the practitioner.
- Continuously evaluate laboratory results, diagnostic tests, and therapy regimen.
- Coordinate appropriate laboratory and diagnostic testing.
- Collect required data and utilize data to adjust the treatment plan when indicated, under the direction of the provider.
- Comply with the Springfield Clinic incident reporting policy and procedure.
- Maintain required documentation.
- Adhere to HIPAA guidelines.
- Assist other team members to ensure even distribution of workload and shares in assigned tasks.
- Incorporate excellent written, verbal, and listening communication skills, positive relationship building skills, and critical analysis skills.
- Adhere to all OSHA and Springfield Clinic training & accomplishments as required per policy.
- Provide excellent customer service and adhere to Springfield Clinic's Code of Conduct and Ethics Standards.
- Perform other job duties as assigned.
Education/Experience
- Associates or Bachelor’s degree from an accredited Nursing Program.
- Two years of clinical experience required, preferably in Infectious Disease.
- Two years of case management experience preferred.
Licenses/Certificates
- RN license in the State of IL required.
- CPR certification per American Heart Association guidelines required within 30 days of hire.
Knowledge, Skills and Abilities
- Highly motivated and self-directed individual that also participates well in the overall team environment.
- Excellent oral and written communication skills.
- Demonstrates experience with program development/program management and workflow assessment including data collection and review.
Working Environment
- This job operates in a professional office environment.
- Routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
