Summary: Tapestry 360 Health is a Federally Qualified Health Center (FQHC) committed to advancing health equity and delivering high-quality, compassionate care to our diverse Chicago communities. The Program Coordinator, Title X Illinois Family Planning Program plays a vital role in ensuring that patients have access to comprehensive, culturally responsive family planning services. This is a grant-funded position responsible for the day-to-day operations of the Title X program. The Coordinator ensures compliance with federal and state guidelines, supports providers and staff, and partners with the Illinois Department of Public Health (IDPH) to meet program goals. Essential Duties and Responsibilities:
- Provide daily oversight of Title X program activities to ensure compliance with federal and state funding guidelines.
- Review and update program policies and protocols as needed.
- Maintain effective relationships with IDPH funders and partner clinics.
- Coordinate required reporting and assist with grant applications.
- Support onboarding of new staff and deliver annual Title X program orientation.
- Monitor billing processes to ensure adherence to program requirements.
- Manage inventory, ordering, and re-stocking of Title X medications and supplies.
- Collaborate with outreach and marketing teams on program visibility.
- Attend IDPH subcommittee meetings, reproductive health conferences, and site visits.
- Provide technical assistance to clinic staff and ensure smooth program operations.
Qualifications:
Required:
- Associate’s Degree in social services, health-related field, or equivalent experience.
- Strong communication and organizational skills.
- Ability to work effectively with diverse communities and practice cultural humility.
Preferred:
- Experience in reproductive health, Title X, or community health settings.
- Prior work with federally or state-funded grant programs.
- Bilingual in Spanish.

Position Details
- Position Type/Hours: Full-time, 40 hours per week (some evenings and weekends required).
- Grant-Funded Position: Continuation of this role is contingent upon ongoing grant funding.
- Travel: May be required for outreach, conferences, and inter-site program support.
Salary and Benefits:
- Compensation: The annual salary range for this position is $40,000 to $58,000, based on experience and qualifications.

This range represents the potential compensation for the role based on experience, tenure, and performance over time.
Most new employees can expect an initial offer within the lower portion of the range, reflecting factors such as prior experience, internal equity, and organizational budget. Salary progression is evaluated regularly to support professional growth and retention.
