What You’ll Do

• Participate as an active member of the interdisciplinary team, attending assessments and daily team meetings to share updates and advocate for participants.
• Develop personalized care plans after assessing participant needs for in-home care provided by Home Care Aides and/or      Specialists.
• Deliver skilled nursing visits under physician orders.
• Respond to participant and family concerns with compassion and professionalism.
• Contribute to quality improvement initiatives and maintain accurate documentation.

Requirements

What We’re Looking For

• Licensure: Current RN or LPN license in the State of Oklahoma.
• Experience: Minimum 2 years of nursing experience (wound care preferred) and at least 1 year working with frail or elderly populations.
• Skills: Strong assessment abilities, organizational skills, and the ability to collaborate across diverse teams.
• Other: Valid driver’s license, proof of insurance, reliable personal vehicle, and a clean driving record.
