The Social Worker provides educational / support services to patients, families, and staff. The Social Worker also provides discharge planning, crisis intervention and information / referral / coordination services to inpatients. The social worker is also involved with patient advocacy, risk management and ethical concerns in relation to the provision of direct services. On-call responsibilities may be required.

Qualifications:- Education: 4 Year / Bachelors Degree in Social Work, required.
- Experience: Minimum two years professional Social Work experience, preferably hospital / health related, required.
- Additional Skills (required): Ability to drive/travel to multiple facilities/locations as needed
Use and analyze patient history assessments and/or clinical data to identify individual LVAD patient needs. Assess current treatment, as well as aftercare needs. Completes comprehensive psychosocial assessments to include evaluation of the patient/family social, emotional, financial needs.
Works collaboratively with each patient and the LVAD team to execute the safe discharge plan established based on patient needs and available resources.
Utilizes knowledge of various levels of care and current community resources to connect each patient with appropriate support in the community to best promote ongoing recovery. Secures aftercare appointments, verifies living arrangements, and facilitates placement
in other long-term facilities as needed.
Responsible for program development, implementation and evaluation of support groups for the LVAD patients and their families/caretakers.
Responsible for developing the support group agendas, time frames and other aspects of the support groups.
Provide and receive input about patient billing, finances, and/or insurances. Adheres to Novant Health policies and procedures related to patient confidentiality, information sharing and patient rights
Communicates individualized plan of care and patient's progress toward goals to all members of the care team including the patient and his/her chosen support system/legal guardian/care coordinator
Escalates complex cases and engages in complex case meetings involving outside departments for guidance in patient care and discharge coordination.
Teamwork: Develops and maintains positive relationships with team members, leadership and community providers.
Resolves conflicts independently. Accepts responsibility for actions and responds positively from feedback. Supports team members related to occupational stress.
