About CommuniCare+OLE Established in 2023, CommuniCare+OLE is the result of a union of two health centers with deep roots in their respective communities and reputations for providing high-quality primary care to all, regardless of insurance or ability to pay: OLE Health of Napa and Solano Counties and CommuniCare Health Centers of Yolo County. Building on a legacy established by both organizations in 1972, CommuniCare+OLE is a network of federally-qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. Many services are offered outside of its sites, including mobile health, home visiting, and community and school-based programs.

JOB SUMMARY/OVERVIEW: The Youth and Family Specialist for Youth Services provides outreach, engagement, behavioral support and transition services at clinic sites, as well as outreach locations throughout Yolo County, to include home visits and work in schools. | |||

MINIMUM POSITION REQUIREMENTS: EDUCATION, EXPERIENCE, SKILLS/TRAINING

| A minimum of 1 year experience working in a mental health setting required. |

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
1. | Conducts screening and intakes for clients and families referred to Youth Services.
|
2. | Provides skill based and rehabilitative behavioral interventions to clients throughout Yolo County in clinic, in the community, in home and in schools.
|
3. | Participates in the family-centered, strength based, culturally relevant, needs driven planning process for youth and families referred to Youth Services.
|
4. | Coordinates and facilitates Child and Family Team meetings with the client, family, natural supports, and formal supports/professionals.
|
5. | Works with the school staff, youth, and their family to facilitate safety planning and individualized youth and family plan.
|
6. | Utilizes evidence-based assessment and outcome measurements tools, as well as compiles and reports data.
|
7. | Completes all client information and documentation in a timely manner (48 hours) according to guidelines.
|
8. | Works with clients to establish long and short-term goals. Specialist is responsible for detecting emotional, psychological, and physical danger signals. Specialist is non-judgmental and offers feedback and support to clients.
|
9. | Responsible for case management, crisis intervention - including de-escalation - safety planning and collateral services for youth and parents/caregivers.
|
10. | Provides direct services to clients and families, providing skills and support to achieve individualized youth and family goals.
|
11. | Maintains a 65%+ level of productivity of direct client service.
|
12. | Responsible for remaining within the ethical standards established in the field, including those of confidentiality.
|
13. | Works as a part of a multi-disciplinary team including Probation, Juvenile Detention Facility staff, CWS, behavioral health clinicians, psychiatrist, youth and parent partners, and other county agencies to coordinate care for youth and families as directed by supervisor.

14. | Attends ongoing supervision and weekly staff meetings. Attend training/conferences as arranged.
|
15. | Responsible for maintaining the standards as listed in the CommuniCare+OLE Personnel Policies and Procedures Manual, and any other standards which may apply to this position.
|
16. | Performs other duties as assigned. |
