Who We Are. We are a collaborative community practice, with locations in Fullerton, Anaheim, Orange, and the High Desert. We believe that all should have access to high quality comprehensive medical care, regardless of ability to pay. Our health centers use a team-based model of care that includes primary care, family medicine, obstetrics, women’s health, oral and vision care, preventative and educational health, mental health, patient advocacy, case management, specialty care, and community health. Most of our patients are at or below 200% Federal Poverty levels, best served in a language other than English. Through our origins with Providence St. Joseph Health, we have provided care to low-income families since the 1990s, maintaining a commitment to serving our most vulnerable neighbors with high-quality, nationally recognized care. We are seeking an eager and dedicated Family Practice Nurse Practitioner at our outpatient/ambulatory clinic in Fullerton.

  • Preferably bilingual (English/Spanish)
  • Committed to providing high-quality care to underserved patients in Fullerton, CA.
  • Adult primary care and chronic disease management comprises the bulk of care.
  • Responsible for assessment, diagnosis, and management of healthcare needs, utilizing standardized and evidenced based procedures.
  • This position reports to the Nurse Practitioner Supervisor.
  • St. Jude Neighborhood Health Centers utilizes EPIC for our Electronic Medical Records (EMR) system.
  • Ambulatory (outpatient) and mobile clinic practice.
  • Daytime schedule
  • No-after hours

Your Working Environment & Team. Working hours are Monday through Saturday, 8AM – 5PM. The position does not currently involve after-hours call duties. It does require working one Saturday per month for which a weekday is flexed off. You would be joining a team of family physicians, nurse practitioners, RNs, pediatricians, OB-GYNs, dentists, specialists (optometrist, podiatrist, dermatologist), licensed social workers, mental health professionals, case managers, health educators, registered dietitians, LVNs, and promotoras to mitigate social determinants of health.

Salary:

$124,000 to $158,000 DOE

Requirements

Education:
Masters Degree an accredited Nurse Practitioner program.


License/ Certifications:

  • Certification from an accredited Nurse Practitioner program.
  • Valid Nurse Practitioner Furnishing number issued by the California Board of Registered Nurses.
  • Valid California Registered Nurse/ Nurse Practitioner license.
  • Provider level basic life support (BLS) from AHA approved program.
  • DEA Certification.
  • Drivers' License in good standing with clean driving record.

Experience:

  • Minimum of 6 months in an ambulatory or community setting
  • Experience in a Federally Qualified Health Center (FQHC), preferred

Knowledge/Skills/Abilities:

  • Proficiency in EMR Systems
  • Proficient in computer programs and systems, including email, internet, and Microsoft Office suite
  • Knowledgeable in applying evidenced-based medicine to treat and diagnosis. Demonstrated ability in applying methods of scientific inquiry and exercising of clinical judgment in challenging situations.
  • Demonstrates the ability to read, write and speak English fluently.
  • Demonstrates the ability to read, write and speak English and Spanish fluently without an interpreter, preferred.
  • Commitment to continuous learning and professional development, remaining current on medical practices and professional credentialing and licensure necessary for continuous employment with St. Jude Neighborhood Health Centers.
  • Demonstrates interpersonal/cultural sensitivity and work with a diverse population.
