Position Summary
The RN Case Manager coordinates healthcare delivery and discharge planning with the multidisciplinary team. This role ensures effective communication between providers, patients, families, and payer sources, promoting high-quality care that is also cost-effective. From admission to discharge, the Case Manager supports care coordination, patient advocacy, and continuity of services in the community.
Key Responsibilities
Lead treatment team and admission conferences with patients and families.
Contact external payor sources upon admission and provide ongoing clinical updates.
Initiate discharge planning at admission and adjust as patient/family needs evolve.
Collaborate with physicians, therapists, and nurses to ensure treatment aligns with patient goals.
Provide clinical documentation to support continued stays and re-authorizations.
Resolve patient/family concerns and ensure positive relationships with all stakeholders.
Coordinate community services and resources to ensure safe and effective discharges.
Maintain timely, accurate medical records of treatment progress, discharge plans, and payor interactions.
Qualifications
Education: Bachelor’s degree in healthcare preferred.
Licensure/Certification:
Current Texas RN license (or compact) or Social Worker license.
Current AHA BLS certification.
Experience:
3–5 years of direct pediatric patient care required.
Pediatric case management experience strongly preferred.
Strong background in healthcare coordination and discharge planning.
