Description

Location is in Wayne and Ecorse

Purpose: The Registered Nurse, in coordination with the provider(s), functions as a direct resource for patients coordinating services with other medical facilities and Hospitals for complete care.

PART I: ESSENTIAL POSITION FUNCTION AND DUTIES

1. Functions are primary partner with Provider providing direct care to patients.

2. Coordinates education and service delivery to patients internally.

3. Based on assessment, provide the patient appropriate direct care and education.

4. Refers to or consults with other health providers or resources based on assessment in order to prevent or resolve problems or concerns.

5. Works with the Quality Coordinator auditing and monitoring meaningful uses measures, HEDIS performance and other quality issues.

6. Follows universal precautions.

7. Establishes an interpersonal relationship with individuals or families intended to increase or enhance their capacity for self-caring and coping.

8. Seeks information and generates optional solutions to perceived problems or issues through interactive problem solving with individuals.

9. Participates as a member of a multidisciplinary team to facilitate teamwork, improve patient care processes and outcomes, and maintain an orderly work environment with efficient clinic flow.

10. Responds to crisis situations according to policy.

11. Demonstrates communication skills that are clear, effective, facilitate patient care and team functioning, and is respectful of differences.

12. Supports compliance and Principles of Responsibility, maintains confidentiality, and protects organizational assets.

13. Exhibits ethics and integrity and adheres to applicable federal and state laws and regulations, accreditation and licensing requirements, memorandums of agreements, policies and procedures; reports and/or resolves issues of non-compliance.

14. Assists in the selection, training, and orientation of Medical Assistants as required

15. Performs assessments of completeness of care; work cooperatively with providers and health staff to ensure completeness of care.

16. Performs other duties as assigned.

 

Qualifications


 

PART III: EDUCATION, EXPERIENCE

1. Current license as a Registered Nurse required

2. Current Basic Life Support certification required.

PART IV: KNOWLEDGE, SKILLS AND ABILITIES

 

1. Knowledge of general nursing principles and practices

2. Knowledge of current nursing technology and methods

3. Skill in the application of nursing techniques

4. Ability to assess the effectiveness of nursing care and to recommend changes to nursing care programs and methods

5. Ability to administer medications and implement treatment plans

6. Ability to instruct others on promotion of health and prevention of disease

7. Ability to provide nursing care direction to medical assistants

8. Ability to maintain composure under stressful conditions

9. Ability to maintain accurate records and prepare reports and correspondence related to the work

10. Ability to maintain favorable public relations

11. Skilled with working in a complex work environment

12. Ability to maintain confidentiality at all times and maintain organizationally appropriate relationships

13. Knowledge of organization policies and procedures.

14. Skilled in exercising initiative, appropriate judgment, problem-solving and decision making.

15. Skilled in developing and maintaining effective relationships with program participants
