Our client needs an RN Case Manager to deliver field-based hospice care in Cameron County, Texas. This role is part of a growing hospice group expanding across South Texas. They want a steady, compassionate nurse ready to own patient care in the community.

What you’ll do

• Deliver hospice care in patient homes and facilities

• Manage your caseload with clear ownership

• Coordinate with care team, patients, and families

• Document visits accurately and on time

• Rotate on-call (extra pay)

• Handle occasional after-hours visits (per-visit pay)

Must-haves

• Active RN license in Texas

• Hospice experience helpful (not required)

• Strong clinical and fieldwork comfort

• Reliable and compassionate with patients

• Willing to cover on-call and extra visits

Perks & pay

• $70,000–$80,000 base salary
