Requirements

Key Responsibilities:

• Conduct initial and ongoing assessments of patients' physical, emotional, and psychosocial needs
• Develop, implement, and revise individualized care plans based on patient and family input
• Ensure compliance with Hospice policies and regulations, including Medicare requirements
• Provide education and support to patients and families regarding disease processes and end-of-life care
• Document all patient interactions and care updates accurately and in a timely manner
• Supervise and support other nursing staff and paraprofessionals in patient care
• Participate in interdisciplinary team meetings and case conferences
• Contribute to quality improvement initiatives within the Hospice program
• Maintain strong relationships with patients, caregivers, and healthcare providers
