BHC is seeking a focused and adaptable Care Manager to join our dynamic Home Care Leadership team in Nashville, TN. The ideal candidate will thrive in a fast-paced environment, possess excellence organizational skills and be able to handle a variety of tasks with precision. Responsible for ensuring services operate safely and efficiently within the requirements of the state and the agency, while providing and instilling excellent customer service to all clients, families, and staff. Every duty or responsibility should further the mission of BHC and improve the quality of life for every client receiving our services.
QUALIFICATIONS:
- Education: High School Diploma or GED, College Degree preferred
- Licensure: Must have a valid driver’s license.
- Experience: Home Care or Senior Care industry experience required.
- Transportation: Personal & reliable transportation. Valid and current auto insurance.
Salary:
- 50,000 Annually
SKILLS:
- Ability to lead and motivate while providing direction.
- Experience with Microsoft Office and virtual meeting platforms.
- Outstanding customer service skills.
- Excellent verbal and written communication skills to effectively interact with employees, clients, and senior leadership.
- Ability to identify issues, analyze data, and develop solutions to improve scheduling efficiency.
- Ability to communication and pivot daily activities, responsive to the needs of the current and future employees.
- Great at relationship building.
- Proficient in all technology platforms and has ability to learn software programs quickly and efficiently.
RESPONSIBILITIES:
- Establish and maintain strong, positive relationships with clients and their families.
- Always serve as the primary point of contact for clients and families.
- Maintain routine check-ins with clients to ensure they are receiving the care they need and to address any concerns.
- Develop/update client care plans when a change in condition occurs
- Onboard new caregivers, ensuring they understand client-specific needs and are properly trained.
- Forecast hiring needs and communicate needs with Recruiter.
- Create and manage the caregiver schedule, meeting the needs of the clients with the most viable caregiver.
- Anticipate possible gaps in the schedule and resourcefully and proactively find solutions so that all clients get the services desired.
- Complete supervisory visits semi-annually on all clients to evaluate their ongoing needs and satisfaction with care services
- Conduct first-shift visits to introduce caregivers to clients and provide initial training and guidance on care needs.
- Monitor caregiver performance and provide guidance, feedback, and support to leadership team.
- Maintain accurate records of client documents (E.g. client agreements, care plans, advance directives, med lists, and caregiver documentation).
- Maintain clear, concise, and up-to-date communication with Executive Director.
- Participate in an on-call rotation.
- Potentially fill in on open shifts that need coverage to call outs and/or staffing shortages.
