Swope Health is seeking a dedicated, and mission-driven Dietician to support a team of caregivers the provide care for the people in our community who need it the most. The Dietician will provide monthly nutritional assessment and case review for all clients assigned, to assess participant’s current nutritional and health status, current interventions and plan of care, and provide recommendations and assistance in the development of specific lifestyle modification, dietary, and treatment plans to support improvements in the current health state and conditions of clients served. This role is a hybrid role that is primarily virtual, with scheduled onsite time as indicated, to provide direct client education or interventions specific to dietitian services.
Primary Responsibilities:
- Lead groups of a therapeutic or psycho-educational nature as part of treatment services.
- Provide ongoing treatment services as described in the program's procedure manual to target populations
- Serves as part of the Specialized Health Care Consultant role, as assigned, for the Behavioral Health- Health Care Home program.
- Provides dietitian counseling and education to BH HCH participants and caregivers as indicated to support treatment and health improvement goals.
- Conducts yearly client case reviews, each assessment 1 hr. min. per BH HCH participant, as assigned by program director to review current interventions and plan of care and to determine additional recommendations for treatment interventions or necessary changes that will promote progress for the client into improve ranges for weight, laboratory values, and overall assist the client toward goals for health improvement.
- Develops therapeutic diets and nutritional strategies for clients served, that will promote progress toward improved health for a variety of medical conditions, address environmental and lifestyle barriers and challenges, and will provides on as needed basis the education necessary for participants to implement diets and strategies to nutrition and dietetics standards.
- May provide as needed, individual or group nutrition education and will submit documentation of the activity and services provided in the client record.
- Maintains up-to-date and appropriate documentation in the participant's electronic medical record, program spreadsheets, and/or any other form of documentation required to reflect dietitian services provided to participants.
- Participates in the interdisciplinary treatment team meetings, communicating participant changes, collaborating on care planning decisions and coordination of care and service delivery.
- Participates in in-service training and staff meetings as required.
- Performs other duties as assigned
- Assists in providing in-service education for the BH associates.
Education/Qualifications
- Master’s Degree from a dietetics program accredited by the Accreditation Council for Education in Nutrition and Dietetics (ACEND)
- Registered Dietician (RD) or Registered Dietician Nutritionist (RDN) -An individual meeting the registration requirements established by the Missouri Commission of Dietetic Registration (CDR) and licensed by the Missouri Division of Professional registration
- Knowledge in the principles and practices of nutritional counseling and education.
- (1 to 2) years' experience working as a Registered Dietitian.
- Working at least 1 year with clients with a co-existing mental health condition preferred
