Population Health Manager (FT) | Population Health | Ames | 2025-190 presso McFarland Clinic

McFarland Clinic is currently accepting application for a Population Health Manager for its Ames office. Candidates should be service-oriented, a team player, and be able to provide extraordinary care, every day to our patients.

GENERAL SUMMARY OF DUTIES: Responsible for the coordination and management of Population Health Department, including the development and implementation of standardized workflows and care pathways that support the clinic's quality and financial goals for value-based care initiatives.  Management and other duties include staff supervision and development, budgeting, planning departmental projects and priorities, and assisting with care management as needed, all in accordance with McFarland Clinic’s Employee-Level and Management-Level Vision Related Behaviors.  Serves as a resource to physicians and staff in other clinical departments to meet quality and financial goals related to value-based care.

Education

• Graduate of an accredited school of nursing or an appropriate level of clinical training. 

Certification/License

• Current State of Iowa Registered Nurse license.
• Possession of a State Practical Nurse license. 
• Certification or registration from an approved certifying organization for Medical Assistants (AAMA, ARMA, AMT, NAHP, NCCT, NHA). 
• Current Basic Life Support certification for Health Care Providers. 
• Mandatory Reporting of Dependent Adult and Child Abuse

Days: Monday - Friday

Hours: 8:00 AM - 5:00 PM. 

Experience

• Minimum of five years' experience in a field of qualified study. 
• Experience with population health, care coordination, disease management, and/or value-based care preferred. 
• Previous management experience preferred.
