Description

The Program Coordinator reports to the Qualified Intellectual Disability Professional and manages the staff and residences of a caseload of 10-15 people supported.  Caseload assignments will include Intermediate Care Facilities (ICFs), Waiver, or some combination of both.  The Program Coordinator must commit to oversight of all facility operations on twenty-four (24) hour/seven (7) days a week basis either in person or on call through a company-issued cell phone. As a Program Coordinator, you are the delegated authority for each assigned location and have the responsibility and accountability to carry out all assigned duties. The Program Coordinator spends 60% of his/her time on person-centered Direct Services and Support and 40% of his/her time on administrative duties, training and documentation.

The Program Coordinator is expected to Interact frequently and positively with persons supported, external professionals, clinicians, family, and the community at large.  The Program Coordinator also has a responsibility to report all allegations or suspicions of abuse, neglect, or violations of the person supported’s “Bill of Rights”, in accordance with RCM’s policy for reporting.

Essential Functions:

STAFFING:

  • Manages a team of direct care staff for residents in assigned locations.
  • Develops and maintains staffing schedules in the scheduling system for assigned locations and ensures that appropriate staffing ratios are being maintained at all times. Schedules coverage for staff absences. Ensures that there is adequate staffing coverage for medical appointments, outings, and medication administration duties.
  • Performs administrative tasks in the web-based payroll system including entering and distributing weekly schedules, reviewing and approving timesheets, approving requests for leave.
  • Assists QIDP with identifying skills gaps and coordinating staff development and training.
  • Completes performance assessments for assigned staff on an annual basis, minimally, and more frequently if needed. Counsels direct staff when performance deficiencies exist and initiates disciplinary procedures as needed under the guidance of Human Resources.
  • Notifies Human Resources of staffing needs for open, permanent shifts.  Interviews, selects, and trains new staff.

HOUSE MANAGEMENT:

  • Ensures that all assigned locations meet the needs of its residents and complies with local, state and federal codes, company policies, procedures and regulations outlined in licensure regulations and standards.
    • Performs environmental checks at regular intervals to ensure that each assigned location is in compliance as stated above.
    • Communicates maintenance, repair, and aesthetic needs to the Operations team and ensures that the requested services are satisfactorily and fully completed.
  • Maintains an inventory of supplies, including grocery and cleaning supplies, essential to the daily operation of each assigned location.
  • Facilitates usage of assigned RCM of Washington vehicles including training new staff on vehicles, notifying Operations of new drivers, ensuring staff is following proper safe driving practices and reporting accidents and issues with the vehicles immediately and per policy.
    • Ensures that the upkeep and driving log of the vehicles are kept up to date.
    • Facilitates Geotab distribution and access with Operations.
  • Coordinates monthly staff house meetings and attends weekly management team meetings. Prepares required documentation of monthly staff house meetings.

PROGRAM COORDINATION:

  • Oversees financial accounts of the persons supported, including assisting with banking and being responsible for their deposits and withdrawals. Responsible for collecting and organizing receipts for all transactions and uploading those receipts as part of the monthly financial reconciliation process.
  • Assists in preparing court reports and attends hearings as needed.
  • Coordinates with nursing to ensure optimum health of each person in assigned caseload.

DIRECT CARE:

  • Works vacant shifts, as needed.
  • Assists with activities of daily living such as personal care routines, feeding, toileting and dressing.
  • Utilizes adaptive devices (wheelchairs, walkers, hearing aids, etc.) in accordance with given directives and needs of persons which you are serving.
  • Provides medication administration (per DC TME regulations & RCM of Washington Nursing Policy) and reports any deviations or medication errors to the supervising nurse immediately per regulations.
  • Accompanies persons supported on outings and engages them in leisure, educational, and recreational activities.
  • Transports persons supported to and from appointments, therapy sessions, programs, clinics, and other activities as scheduled and/or required.

Qualifications


  • Must possess a high school diploma, GED, or equivalent.
  • Must be able to correspond via email (Outlook) and use web-based applications to perform work, e.g. banking, ordering, file sharing, scheduling, etc.
  • 2 years in a social service setting, preferably in an ICF/Waiver healthcare setting with 2 to 4 years of managerial experience.
  • Possess a valid and current driver license in Virginia, Maryland, or the District of Columbia. Must have reliable, personal transportation.
  • Must be able to pass a criminal background check.
  • Able/willing to navigate the DC community in association with job duties and in accommodation of assigned persons.
  • Have a current and maintain annual health certificate stating employee is free and clear of any and all communicable diseases.
  • Maintain current CPR and First Aid Certification.
  • Current Trained Medication Employee certification in the District of Columbia strongly preferred.
  • Able and willing to complete duties as directed and model professional behavior.
  • Possess the ability to employ critical thinking skills when circumstances warrant such action (including Crisis Situations)
  • Must have patience, tact, cheerful disposition, and enthusiasm, as well as be willing to handle individuals based on whatever maturity level they are currently functioning. Computer literate with the ability to navigate company established software systems.
  • Ability to lift, move, or assist non-ambulatory persons and/or weights with a minimum of 60 lbs.
  • Excellent organizational skills. Ability to prioritize and have a sense of urgency.
  • Effective oral and written communication in the English language.
  • Ability to gain a working knowledge of RCM of Washington policies and procedures.
  • Ability to act as a representative of RCM of Washington to the public.
  • Excellent interpersonal skills with the ability to effectively communicate and coordinate with other staff members and persons from different backgrounds.
  • Ability to maintain confidentiality.


Physical and Environmental Conditions:
Work environment may involve situations that require working with persons with Behavior Support Plans, Trauma Informed Care Plans, Complex Medical Needs, and Significant Developmental Disabilities.
