About the Role
At Corus Hospice in Albuquerque, we’re seeking an experienced Nurse Practitioner to join our compassionate, mission-driven team. This is a highly autonomous role where you’ll help shape the patient experience while working alongside a collaborative, interdisciplinary care team.

Key Responsibilities

• Conduct in-home or facility-based hospice visits for patients with life-limiting illness
   
• Provide comprehensive assessments, symptom management, and palliative interventions
   
• Collaborate closely with RN Case Managers, Medical Directors, and the broader IDG
   
• Educate patients and families on the hospice journey with clarity and empathy
   
• Certify and recertify eligibility for hospice services in alignment with Medicare guidelines
   
• Support transitions of care and participate in after-hours coverage, as needed
   
• Maintain accurate clinical documentation in accordance with hospice standards
   
• Uphold our commitment to dignity, comfort, and quality of life
   

Requirements

What We’re Looking For

• Licensed Nurse Practitioner (NP) in the state of New Mexico
   
• Minimum of 1–2 years’ hospice or palliative care experience (required)
   
• Strong clinical assessment and critical thinking skills
   
• Comfortable managing care independently in home and facility settings
   
• Compassionate communicator, skilled in sensitive conversations
   
• Flexible, responsive, and calm under pressure
   
• EMR proficiency and reliable transportation
