What You'll Do
As a Family Resource Specialist, you will provide direct support to patients and families in addressing social needs, working collaboratively with community partners and care team members to close gaps related to non-medical drivers of health. In this role, you will assist in building and strengthening community relationships, identifying and maintaining a robust
network of resources, and offering non-clinical support, guidance, and education to families navigating the unique challenges of caring for children with complex medical and behavioral health needs. In this position, you will:
- Identify resources and community support available to the family or caregivers
- Provide information, referrals, and non-clinical assistance with linkage to medical, psychosocial, and basic needs services
- Empower families to advocate for themselves by providing education, guidance, and supportive tools to navigate systems and access needed services.
- Assist with accessing specific programs and supports to address psycho-social needs both within Imagine Pediatrics and in the larger community
- Advocate with community agencies for, and on behalf, of clients and families to decrease service gaps and remove barriers to services
- Connect with relevant internal staff and providers to assist with system navigation, including eligibility, pediatrics, behavioral health, public health, etc.
- Assist in development of an internal community resource library (but do not create clinical library materials)
- Provide linkage to community-based resources and school-based services
- Network with new and established community resources to increase knowledge of services
- Administratively document memoranda of understanding (MOU) for partnerships with outside community agencies
- Develop effective working relationships with interdisciplinary care teams
- Participate in ongoing scheduled consultations with an interdisciplinary team regarding administrative and non-clinical resources provided to family and caregivers
- Perform other duties as assigned
What You Bring and How You Qualify
First and foremost, you’re passionate and committed to reimagining pediatric health care and creating a world where every child with complex medical conditions gets the care and support they deserve. In this role you will need:
- Bachelor’s degree or higher in healthcare, behavioral health, or social services related field preferred
- Certification as a Community Health Worker highly desired
- Familiar with navigating social service agencies
- Must have reliable transportation, a valid Driver's License, and ability to produce a copy of current auto insurance certificate
- Experience with resourcing and telephonic services
- Bilingual Spanish preferred
- Work hours are from 8am-5pm Monday-Friday

The hourly rate for this position ranges from $25-30/hr in addition to competitive company benefits package and eligibility to participate in an employee equity purchase program (as applicable). When determining compensation, we analyze and carefully consider several factors including job-related knowledge, skills and experience. These considerations may cause your compensation to vary.
