Title: Clinical Nurse Coordinator
Department: Recovery and Wellness Center Medical/Dental Unit
Reports To: Senior Nurse Manager
Summary:
The Clinical Nurse Coordinator will oversee all clinical support at Recovery and Wellness Center (RWC) medical/dental unit, including direct patient care, clinical and administrative tasks associated with day-to-day functions at RWC. The Clinical Nurse Coordinator will provide outstanding patient experience, and efficient clinical operations. The Clinical Nurse Coordinator should have a track record of creating a positive and encouraging work environment for those with whom he/she has worked.
Essential Duties and Responsibilities:
- Manage all clinical support of the RWC medical/dental unit including but not limited to direct patient care, nursing visits, assisting with procedures, establishing templates and coordinating provider schedules.
- Performs technical skills, seeking assistance as needed, according to established procedures, protocols and scope of nursing practice, care standards
- Utilizes knowledge of dosage calculations, indications, actions, routes, and side effects for proper administration of medications. Obtains appropriate consents before administration of vaccines/other medications as appropriate
- Exhibits an understanding of infection control principles and universal precautions when changing sterile dressings, injections, or performing any procedure that required sterile techniques
- Evaluates and documents the effectiveness of Nursing intervention and teaching by patient’s response to teaching measures, behavior outcomes, knowledge base/understanding and/or skill demonstration and ongoing reinforcement of educational process
- Performs complete documentation responsibilities relating to patient's medical records in a timely, accurate and concise manner
- Order medical supplies and services as needed to ensure proper functioning of the medical team.
- Works with medical/dental providers to achieve quality of care and operational goals of the RWC medical/dental unit.
- Able to facilitate psycho-education groups as needed (focus on health and wellness, medication education, side effects, adherence strategies)
- Develops, individualizes, and professionally modifies care through physical, psychosocial and cognitive realms of nursing assessment and interventions actively considered
- Responsible for performing face-to-face shelter intakes and phone triage. Assesses patients’ medical urgency and determines care needs based upon presented symptoms and department standards of care
- Performs vision tests, Urine Dipsticks/Urinalysis, Urine Pregnancy Tests and other CLIA waived tests when necessary.
- Ensures all instruments and supplies are used properly and sterilized and stored for transport per infection prevention protocol.
- Maintains supply inventory.
- Monitors maintenance of accurate logs including but not limited to point of care testing, controls, expiration dates, refrigerators and freezers.
- Ensure that the necessary equipment and information are available for exams.
- Maintains regulatory readiness and participates in monthly EOC rounds in collaboration with other departments on site/per company protocol.
- Ensure compliance with infection-control policies and protocols.
- Complete required data collection and generate timely reports.
- Attend and participate in departmental meetings and training sessions.
- May load and unload equipment as necessary.
- Proficient in navigation and management of multiple EMR and time management systems.
- Responsible for assuming other duties as may be necessary for the efficient and effective management of CS-HHC operations.
- Leads Quality Improvement (QI) projects for RWC medical/dental unit.
- Provide supervision, oversight, and collaboration of RWC medical /dental unit site coordinator.
- Assist in standardization, development, and maintenance of competencies and clinical workflows.
- Participate in any relevant QA/QI or Regulatory agency wide meetings/committees/audits, etc. as appropriate. (For example: EOC Rounds, QA/QI, Regulatory Readiness Committee)
- Assist in education of staff on grant expectations and requirements and ensure compliance.
- Collaborate with regulatory/compliance teams to ensure unit meets all regulatory and patient care standards; must be able to identify and address any quality-assurance and compliance issues.
- Works collaboratively with interdisciplinary teams.
- Perform billable nurse visits where applicable according to established protocols.
- Can work autonomously within the field of practice, creating an encouraging and supportive relationship.
- Completes government documentation as required by the Grants served
- Responsible for assuming other duties as may be necessary for the efficient and effective management of CS-HHC clinical operations.
Qualifications
Education: Must have a valid CT RN license. Current BLS required.
Experience: Two to four years of nursing experience required with psychiatric and or addiction medicine experience strongly preferred.
Knowledge/Abilities: The Clinical Nursing Coordinator will have demonstrated excellence in a clinical nursing setting, preferably in primary care, and will ideally have experience with leadership, staffing, or quality improvement experience. Organization, flexibility, and effective communication skills are critical. The Clinical Nursing Coordinator should be able to interact effectively with all levels of staff and to create around him or herself a collaborative and supportive team atmosphere. The Clinical Nursing Coordinator will have demonstrated ability to identify and initiate departmental process improvements and set pertinent clinical goals for homeless healthcare department.
