- Conduct comprehensive initial and ongoing assessments of patients and families to determine hospice care needs.
- Develop, implement, and adjust individualized care plans based on patient conditions and evolving needs.
- Administer medications, treatments, and palliative care interventions as prescribed.
- Educate patients and families on symptom management, medication use, and self-care techniques.
- Provide emotional and psychological support to patients and their loved ones.
- Assist with activities of daily living to enhance patient comfort and independence.
- Serve as a Case Manager, coordinating care for assigned patients.
- Maintain accurate, detailed clinical records and document changes in patient conditions.
- Collaborate with physicians, caregivers, and interdisciplinary team members to ensure seamless care.
- Participate in family and team meetings, providing updates and guidance on care plans.
- Advocate for patient needs and facilitate access to necessary resources and support services.
- Participate in on-call rotations as needed.
- Ensure necessary medical equipment and supplies are available for patient care.
- Supervise and support hospice aides as needed.
- Engage in professional development and continuing education.
- Contribute to quality improvement initiatives within the organization.
- Graduate of an accredited School of Nursing
- Active RN license in the state of Minnesota
- CPR certification required
- Minimum of 1 year of acute care experience (institutional setting) and 1 year of home care or hospice experience preferred
- Strong clinical assessment, critical thinking, and problem-solving skills
- Compassionate and patient-centered approach to care
- Ability to lift, position, and transfer patients as needed
- Must have a valid driver's license and insured vehicle for travel to patient locations
