The incumbent will assist with patient transition processes, EMR evaluation, and will assist the care team with other processes such as electronic form routing and patient identification. Assist patients with scheduling post SNF discharge visits with providers, therapy, Case Managers, etc. Assist the PostAcute Case Managers in ensuring that patients have all discharge information to promote successful transition to next level of care. Healthcare and health systems are complex and often confusing entities for patients today. will help patients participate in their own care and successfully navigate through the respective Health System Partner organizations.
