We have an opening in our Old Orchard Beach (OOB) extended care program for a Part Time Residential Case Manager. 

Milestone Recovery is a low barrier facility that allows clients to access services no matter the status of their insurance coverage. This allows staff at Milestone Recovery to function in a more client-centered way. The voices of our staff are greatly valued in implementing changes, encouraging improvements, and promoting the overall care and wellbeing of the clients whom Milestone Recovery serves.? 

Position Profile: Our Residential Case Manager works directly with people in addiction (substance use disorder) treatment through their early reentry phase of Milestone’s Extended Care Program. This work may also involve families and friends of the client. The Residential Case Manager also works with service providers, to engage resources and achieve coordination of care for a client.

Residential Case Managers do not focus on clinical work, but they must still be capable of responding in professional, ethical ways, including cultural responsiveness and trauma-informed best practices.

Essential Functions and Responsibilities:

• Collaborates with the client on a needs assessment to identify areas of focus.
• Assist clients in accessing and maintaining needed services to support clients’ community integration efforts and plan.
• Links to and provides support with other services (vocational, educational, social/recreational, transportation, etc.) and advocates for entitlements such as MaineCare.
• Facilitate ongoing collaboration and communication between providers.
• Integrates clients supports (family, social, community) to help remove barriers for clients.
• Complete necessary administrative tasks according to deadlines.
• Attends staff meetings and development training as scheduled.
• Participates in clinical supervision including QA review of documentation and personal goal tracking highlighted in yearly evaluation.
•  Help clients to navigate housing and other resources needed for aftercare.
• Transport clients as necessary.
• Adheres to agency policies and procedures.
• Perform other related duties as assigned.

Work Location and Schedule: This position is located at Milestone’s Extended Care Program in Old Orchard Beach, Maine. This is a 16-hour per week schedule with shifts Wednesday and Friday 8:00 a.m. - 4:00 p.m.

Requirements

 
Qualifications: Valid driver’s license and reliable transportation. Associate’s degree in social services or related field. 1 year of successful community-based mental health/substance abuse work is a plus. Must be MHRT-C certified or provisional eligible.
