Position Summary
The Lead Clinical Liaison RN serves as a Supervisor/Lead Clinical Liaison to the Hub Team(s) and provides leadership and coordination of care for patients and families referred to home health, hospice, and other post-acute services. This role offers day-to-day oversight and support for the liaison team, including RNs and Patient Transition Coordinators, while also functioning in a clinical liaison capacity.
The Lead Clinical Liaison acts as a decision-maker for escalated referrals, ensures best practices in care delivery, maintains positive customer relationships, and promotes Compassus services within assigned accounts. Responsibilities include consultative education, care coordination, and facilitation of patient transitions from acute care to home or alternate care settings. The role also collaborates with leadership to support outreach, training, and growth initiatives.
Position Specific Responsibilities
- • Provide clinical and administrative oversight to liaison staff (RNs and Care Transition Associates) assigned to the hub.
• Lead staff in clinical problem-solving and serve as a decision-maker for escalated patient referrals.
• Coordinate staffing schedules to ensure adequate coverage.
• Lead team meetings to review weekly metrics, resolve service/account issues, and support escalated customer service concerns.
• Respond to daily needs of the local team and facilitate communication between Growth and Intake Departments.
• Ensure regulatory compliance for assigned staff and participate in developing policies and procedures.
• Maintain compliance with Medicare Conditions of Participation, federal/state/local laws, facility policies, HIPAA, and the Patients’ Bill of Rights.
• Maintain confidentiality of patient, company, and client information, ensuring proper handling/disposal of confidential documents.
• Evaluate referrals from hospitals and other care settings for appropriateness for Home Health, Hospice, Palliative Care, and other in-home services.
• Provide patients and families with information about available services.
• Collaborate with discharge planners, inpatient hospital teams, and agency staff to ensure comprehensive care plan development and smooth patient transitions.
• Accept referrals from hospital discharge planners once agency choice and service type are identified.
• Facilitate initiation of Infusion referrals and coordinate discharge planning for Home Health and Hospice patients admitted to hospitals.
• Document activities in hospital EMRs and track referral activity by unit and Compassus agency.
• Maintain and build referral base relationships by serving as an educational and clinical resource.
• Schedule, coordinate, and deliver in-services, community events, health fairs, caregiver series, and other outreach activities.
• Collaborate with Account Representatives and local outreach teams to support referral generation, business growth, and execution of tactical plans.
• Actively participate in development and execution of strategic initiatives to grow Home Health, Hospice, and Palliative Care services.
• Partner with Care Liaison Manager to support tactical plans, outreach activities, and training/orientation of Growth staff.
• Serve as a local champion for service-line standardization and consistency initiatives.
• Support service-line leadership by reporting on tactical plans and growth metrics.
• Perform other duties as assigned.
Education and/or Experience
- Required - Education sufficient for licensure
- Required - Bachelor's degree in Nursing, or a Diploma in Nursing or an Associate's Degree in Nursing (ADN)
- Required - 5 years experience in clinical discipline required.
- Preferred - 3 years of staff supervision experience preferred.
- Preferred - Previous liaison, marketing, sales, or health care industry experience
- Preferred - 3 years home health, hospice and/or palliative care experience preferred.
Skills
- Mathematical Skills:
- Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
- Language Skills:
- Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Demonstrates history of successful clinical and educational presentation skills, sales and marketing results, preferably in healthcare related field. Communicates clearly, sensitively and in a supportive professional manner with patient/family/caregivers, co-workers and public. Strong written and verbal communications in English and excellent customer service communication skills by phone, email, and face-to-face.
- Other Skills and Abilities:
- Ability to articulate and embrace integrated healthcare at home philosophy. Understands the purpose and function of Value-Based Enterprise agreement between Providence and JV for home health and hospice care coordination. Ability to develop positive working relationships with diverse and multicultural staff of ministry, community groups and individuals and applies and maintains strong team collaboration skills within and across ministries and builds strong relationships with operations and clinical team members. Demonstrates strong leadership and supervisory skills; ability to inspire, motivate, and lead clinical team composed of a variety of disciplinary practices, and maintain positive professional development. Demonstrates competency by maintaining positive, collaborative, and constructive interpersonal relationships. Understands and practices the principles of effective teamwork. Demonstrates history of successful clinical educational presentation skills, sales and marketing results, preferably in healthcare related field.
- Demonstrates ability to work with minimum supervisory guidance and exercises independent judgment within the scope of practice; adapts easily to changes in work assignments and environment, can remain calm under pressure and is willing to assume additional responsibility and learn new procedures. Demonstrates ability to prioritize work assignments and meet productivity and quality standards through effective organizational skills and follow-through. Strong skills in facilitating complex problem solving and the ability to work with families/representatives in crisis.
Ability to achieve annual referral and community educational requirements by prioritizing efforts and activities to support increased referral volume and achieve account penetration. Demonstrates flexibility and responsiveness working within a dynamic environment. Ability to follow appropriate communications channels and to travel throughout the service area from site to site and access multi-level buildings.
- Computer Skills:
- Proficient in using computers and knowledge of an Electronic Medical Record (as applicable) and Microsoft Office Suite programs. Knowledge of Electronic Medical Record (Epic) desired. Experience and knowledge of customer relational and territory management software.
Certifications, Licenses, and Registrations
- Required within 30 days of hire: National Provider BLS - American Heart Association (Vendor Managed)
- Required upon hire: Active, unrestricted Registered Nurse (RN) license in all applicable state(s) of practice; multistate or compact license accepted where applicable.
Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
