POSITION SUMMARY:
The Associate Director, Mother/Child Health, Population Health (hereinafter, “Associate Director”) will be responsible for creating and supporting programs and services that maintain quality and stimulate growth for the prenatal and postpartum care services at CHN. The Associate Director plans, implements, and supervises the deliverables and staff of multiple contracts across CHN. This person will collaborate with other team members (i.e., medical, educational, and social services) to coordinate patient care.
JOB FUNCTION:
Responsibilities include, but are not limited to:
- Supervises day-to-day operations of the Mother/Child Health program.
- Interviews, hires and trains Mother/Child Health coordinators.
- Performance evaluations for staff
- Ensure compliance Employee Health Services requirements of subordinates are met
- Oversees grant deliverables.
- Plans and establishes goals for grant-funded programs, including but not limited to, the
- Healthy Start (Public Health Solutions), IMRI (NYC DOHMH) and other Mother/Child Health grants
- Communicates directly with the grant-funding agencies.
- Examines and summarizes data trends of prenatal and postpartum clinical quality and coordination process measures. Provides feedback, and prepares monthly, quarterly, semi-annually, and annual reports
- Develop and implement Plan-Do-Measure-Act (PDMA) cycles to test and refine quality improvement initiatives to ensure data-driven decision-making and measurable outcomes.
- Communicates and collaborates with key leaders at the New York City Department of Health and Mental Hygiene and other state, local, and community health agencies to meet and maintain the standards of health for CHN’s patients as required by these organizations.
- Develops and executes short and long-term goals and programs that enhance the services and growth of Mother/Child Health services for CHN’s patients.
- Plans and coordinates hands-on training and education of clinical staff, nurses, medical assistants, and administrative staff as required.
- Collaborates with interdisciplinary team to increase the number of patients and to ensure access to prenatal and postpartum health services
- Works with the clinical leadership to ensure that best practices of prenatal and postpartum care are incorporated into primary care.
- Coordinates and participates in interdisciplinary meetings that influence policies and practices impacting related to prenatal and postpartum health services.
- Simulates program growth and expansion, plans, implements, and coordinates outreach activities and programs.
- Works with Marketing to create and establish marketing ventures to advertise related services and products of CHN. Develops new linkages with community organizations to increase patient referrals to all CHN facilities and services.
- Works closely with Development to write new grants and complete reports for existing and new grants.
- Participates in the development, revision, and implementation of departmental policies relevant to reproductive and sexual health.
- Represents CHN by attending coalition meetings, community events, trainings, and other relevant events.
- Identifies and attends conferences and relevant trainings to remain up to date in the areas of health care delivery and policy about reproductive health, prenatal and postpartum services.
- Performs other related duties as assigned by supervisor.
WHAT WE LOOK FOR:
- Graduate degree in Public Health, Hospital Administration, or another similar field.
- A minimum of three (3) years of working within a health care setting with a focus on sexual and reproductive health, preferably with supervisory experience.
- Knowledge of prenatal and postpartum or other health information.
