Eminence Home Care CT is looking for dedicated and compassionate healthcare professionals to fill the role of Home Care LPN or RN. In this position, you will provide high-quality, patient-centered care to clients in their own homes, promoting their comfort and independence. As part of our dynamic team, you will work collaboratively with other healthcare providers, families, and clients to develop and implement individualized care plans that meet each client's unique needs.

Responsibilities

• Administer medications and treatments as prescribed by the physician.
• Perform nursing assessments, monitoring vital signs and client condition.
• Assist clients with daily living activities, ensuring their dignity is upheld.
• Educate clients and families on health management and care practices.
• Document patient care and progress accurately and timely.
• Coordinate with healthcare providers to optimize care delivery.
• Provide compassionate support and companionship to clients.

Requirements

• Current and valid LPN or RN license in the state of Connecticut.
• Previous experience in home care, nursing, or a related field preferred.
• Strong clinical skills with the ability to assess and respond to patient needs independently.
• Excellent communication and interpersonal skills to effectively interact with clients and families.
• Ability to work as part of a collaborative healthcare team.
• Reliable transportation and a valid driver's license.
• Ability to perform physically demanding tasks and assist clients with mobility.
• CPR certification required.
