RN - Inpatient Navigator presso Avita Health System

Responsible for navigating patient health care needs in attempts to lower patient risk of hospital readmissions. Provides, coordinates and facilitates patient discharge planning in collaboration with other health care professionals during hospitalization and ED visits. Assists with organizing services across provider lines, between people, and systems to affect optimal patient outcomes, achieve continuity of care and reduce costs.
