Summary: The Engagement Specialist, BHPMW provides CP supports and outreach activities to Assigned Enrollees.

Primary Responsibilities:
- Ensure immediate engagement and contact with all newly assigned enrollees within 72 hours.
- Collaborate with the Team and conduct daily outreach efforts to members assigned to the CP under the supervision of the Clinical Care Manager.
- Conduct all outreach and engagement activities with Assigned Enrollees and collaborate with the Team to schedule in-person contacts with the enrollees for—including but not limited to—scheduling enrollees’ intakes, obtaining member signatures on various eHana documents, and facilitating in-person visits with outside agencies and providers as needed.
- Work collaboratively with all Team members and communicate on enrollee activities in real time.
- Ensure each assigned enrollee is given the opportunity to sign the CP Participation Form or decline to do so. Ensure documentation of this process is clear and documented in eHana.
- Participate in all required orientation and training.
- Learn all BHCP policies, procedures, protocols and plans and deliver CP supports and activities in compliance with them.
- Complete all required documentation in a timely manner.
- Attend and actively participate in supervision and staff meetings.
- Consults with RN, CCM, and other CP Team members as needed around clinical, medical, and other matters.
- Ensure that all clients are treated with dignity and respect in accordance with BHPMW’s Human Rights policy.
- Perform all duties in accordance with the agency’s policies and procedures.
- Strictly follow all agency Performance Standards.
Knowledge and Skill Requirements:
- BA/BS degree in a field related to human services with a minimum of 1 year of experience or High School diploma/GED with a minimum of 3 years of experience working with adults in a community-based and/or medical setting.
- Ability to maintain personal and professional boundaries
- Excellent verbal and written communications skills.
- Strong skills in the areas of communication, follow through, collaboration, and customer service.
- Strong Computer skills proficiency in contemporary Windows operating systems and Windows office suites with an emphasis on Word and Excel; ability to learn new systems; experience entering and working with data; and comfort and experience using mobile technologies.
- Excellent organizational, time management, problem solving skills.
- Ability to openly address and acknowledge issues of substance use and mental illness.
- Knowledge regarding psychiatric rehab and/or substance use and understanding of recovery model.
- Must be able to perform each essential duty satisfactorily.
- Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.
- Must hold a valid drivers’ license. Must have access to an operational and insured vehicle and be willing to use it to drive to member’s homes, medical offices and hospitals, and community settings. Must be willing to spend 100% of time traveling.
- Must have ability to read English and communicate effectively in the primary language of the program to which he/she is assigned.
Organizational Relationship: Directly reports to Clinical Care Manager.

Working Conditions: This facility is a non-smoking environment. As part of the responsibilities of this position, the Engagement Specialist, BHPMW will have direct or incidental contact with clients served by SMOC in various programs funded or administered through the Executive Office of Health and Human Services. A successful background check is required.
Remote Work Option: Remote work is permissible in some positions at SMOC depending on the key functions and responsibilities. The Engagement Specialist, BHPMW position is eligible to work from home --% of the week in scheduling coordination with the department manager.
