What You’ll Do

As Program Coordinator, you will support person-centered, high-quality care by leading the implementation, documentation, and ongoing evaluation of Individualized Program Plans (IPPs) for individuals supported in the Intermediate Care Facility (ICF) environment. You will uphold the integrity of clinical and habilitation objectives, foster collaboration with clinicians and Qualified Intellectual Disabilities Professionals (QIDPs), and ensure full compliance with regulatory standards across all facets of treatment planning and data management.

 

Your Responsibilities Will Include:

Individualized Program Planning (IPP)

  • Reviewing Comprehensive Functional Assessments (CFA) to understand individual strengths, needs, and priorities. 

  • Ensuring IPP goals are developed in alignment with assessments, regulatory requirements, and person-centered outcomes. 

  • Collaborating with clinicians, managers, and QIDPs to create, revise, and implement IPP goals, ensuring goals are SMART (Specific, Measurable, Achievable, Relevant, Time-bound). 

  • Maintaining consistency across all treatment documents, including CFA, IPP Goal Sheets, IPP Data Tracking, and transition binders. 

  • Ensuring IPP goals and data collection sheets are updated, distributed, and collected monthly.

Data Tracking and Reporting

  • Entering and analyzing monthly IPP data in Excel tracking systems within five (5) business days. 

  • Monitoring progression, regression, or maintenance of individual goals and communicate outcomes to clinicians, QIDPs, and managers. 

  • Supporting clinicians in preparing quarterly summaries and ensure documentation is uploaded and incorporated into CFA updates. 

  • Conducting weekly binder and chart reviews to ensure documentation integrity and regulatory compliance.

Quality Assurance & Compliance

  • Auditing documentation levels weekly (shifting to bi-weekly or monthly as compliance improves). 
  • Reporting documentation deficits to managers/principals and maintain accountability through email sign offs and audit logs. 
  • Supporting the organization in maintaining compliance with OPWDD, Bureau of Program Certification (BPC), and internal quality standards. 
  • Ensuring consistency and accuracy across treatment plans, assessments, and data tracking tools.

Collaboration & Communication

  • Holding quarterly meetings with clinicians and managers to review and update IPP goals. 

  • Presenting IPP updates during quarterly and semi-annual CFA/Interdisciplinary Team (ITT) meetings. 

  • Providing updates on IPPs and data integrity during bi-weekly staff debriefings. 

  • Working closely with ARMs, QIDPs, and Senior DSPs to ensure timely replacement of monthly data sheets, especially during holidays or staff absences.

 

You’re a great fit for this role if you have:

  • Bachelor’s degree in Human Services, Psychology, Special Education, or related field (preferred). 

  • Experience in program planning, data tracking, or quality improvement within an OPWDD/ICF setting (preferred). 

  • Strong knowledge of person-centered planning, IPP/ISP processes, and regulatory requirements. 

  • Proficiency in Microsoft Excel and ability to manage large sets of data accurately. 

  • Strong organizational and time management skills; ability to meet strict deadlines. – 

  • Excellent interpersonal and communication skills; able to collaborate across clinical, residential, and administrative teams.

 

Compensation

$23 Per Hour
