We are hiring a Nurse Care Coordinator!

Schedule/Benefits
- Full Time
- Monday-Friday flexible onsite during typical business hours, on call between 6 am-4 pm.
- Full-time, benefits-eligible: Includes medical, dental, and vision coverage, PTO is frontloaded plus accrues, and retirement with company match
What You’ll Do
- Oversee healthcare needs of 16–20 residents in our group homes, ensuring high-quality care in a supportive home environment.
- Coordinate and attend medical appointments, specialty visits, and annual physicals; complete quarterly and PRN health assessments.
- Provide phone support to direct support professionals, medication managers, and CMAs from 6 AM–4 PM; conduct weekly (and as-needed) visits to each group home.
- Maintain medical charts in compliance with state regulations; deliver staff training in areas such as G-tube care, seizure management, and medication administration.
What You’ll Need
- Graduate of an accredited School of Nursing and licensed to practice as a Registered Nurse in the State of Iowa. LPN will be considered if meet other qualifications.
- Nursing experience in the field of developmental disabilities, pediatrics, and behavioral health preferred.
- Must have reliable transportation and be willing to become an authorized driver with ChildServe and use own car to transport children in the community.
- Ability to lift up to 35 lbs, be mobile within the clinic areas, and frequently bend, lift, reach, and stand,
- Active BLS from the American Heart Association or American Red Cross
- Ability to communicate effectively in (or through) reading, writing, and speaking the English language
