POSITION OVERVIEW:
Under the supervision of the Network Director, the Registered Nurse (RN) promotes the physical and mental well-being of people with disabilities and/or mental illness receiving support and services. The RN collaborates with the support team to oversee medical services, including medication administration, ensuring compliance with regulatory requirements while addressing the unique needs of each individual. The RN empowers people to take personal responsibility for their health while ensuring their overall well-being.

 

Salary: $73,000

Location: New Lenox

Hours: Full-time M-F, 8:30am-5:00pm plus on-call

ESSENTIAL FUNCTIONS:

Key responsibilities include, but are not limited to:

 

  • Conduct medical reviews and assessments, including:
    • Health Risk Screening Tool (at admission and annually)
    • Nursing Assessments (annually or as needed)
    • Self-Administration of Medication Assessment (SAMA) (annually and quarterly for individuals with co-occurring diagnoses)
  • Participate in person-centered planning to address healthcare needs.
  • Promote self-care and self-medication practices when appropriate.
  • Educate individuals and staff on medical conditions and healthcare practices; respond after hours for urgent inquiries.
  • Consult with team members and coordinate follow-up care after appointments, labs, or hospital visits.
  • Assist with coordinating care among physicians, clinics, and hospitals as needed.
  • Communicate with families and guardians about medical concerns, as directed.
  • Oversee medical transitions during admissions or hospital discharges and monitor ongoing medication needs.
  • Delegate medication administration tasks to non-licensed personnel.
  • Provide initial and annual retraining in medication administration per Illinois Title 59 Part 116.
  • Maintain accurate Physician Order Sheets (POS) and Medication Administration Records (MARs), ensuring regulatory compliance.
  • Review completed MARs monthly; follow up as needed and report concerns to the Network Director.
  • Address and report injury, illness, and medication error incidents.
  • Document all direct services and instruction according to policy and regulatory standards.
  • Support agency-wide training, including health and safety education, as assigned.
  • Provide RN coverage for medication delegation during scheduled absences and agency needs.
  • Assist in onboarding and orientation of new nursing staff.
  • Commit to ongoing professional development.
  • Attend required trainings and actively participate in nursing/medical meetings.
  • Engage in community outreach and share professional insights with the Network Director.
  • Perform additional duties as assigned.

SKILLS AND ABILITIES:

  • Ability to make independent decisions and solve problems within the scope of licensure and credentials.
  • Strong interpersonal skills with the ability to build rapport with staff, program participants, family members, and external stakeholders.
  • Effective communication skills, both verbal and written, to convey policies, procedures, and medical information clearly.
  • Ability to maintain compliance with agency policies, procedures, and regulatory standards.
  • Competency in reading, analyzing, and interpreting medical data, research, technical procedures, and regulations.
  • Proficiency in calculating medication dosages accurately.

PHYSICAL DEMANDS:

  • Ability to lift up to 40 pounds unassisted.
  • Ability to push and pull up to 75 pounds.
  • Ability to perform deep knee squats and push wheelchairs up to 200 pounds.
  • Ability to stand or sit for prolonged periods.
  • Capability to operate motor vehicles.
  • Physical activities may include driving, balancing, pulling, pushing, standing, climbing, grasping, stooping, crouching, crawling, kneeling, reaching, lifting, and repetitive motion.

QUALIFICATIONS:

  1. Minimum of an Associate’s Degree in Nursing (ADN) and an active Registered Professional Nurse license in the State of Illinois.
  2. Successful completion of the Illinois Department of Human Services (DHS) Nurse-Trainer Training Program.
  3. Preferably, at least one year of experience in nursing, with some exposure to individuals with intellectual/developmental disabilities and mental illness.
