Community Health Worker presso Tower Health

Work Schedule: M-F 8 hour days. This position will work between Phoenixville & Pottstown Hospital

• Community Health Worker will work primarily with specific target populations including but not limited to non-English speaking populations of geographic target areas.
• CHW will work closely with internal and external stakeholders or partners including but not limited to clinical/medical/dental providers, primary care teams, and other agencies to improve patient care and outcomes.
• Patient outreach interactions may be telephonic, video/digital, or in-person generally at an office site.
• Depending on department location CHWs may need to be bilingual or be knowledgeable of specific department terminology such as medical or dental.

Education Requirements

• Completes and maintains all competencies and trainings as required for role.
• High School Diploma - OR - GED - OR - Relevant experience may be considered in lieu of education

Experience

• Non-Profit Work Experience; preferred
• Bilingual in Spanish and/or Portuguese; preferred 

Certification and Licensure

• Current Valid Driver's License
• Certified Community Health Work Certification; Required within 1 year after hire
