Seeking a dynamic and collaborative patient navigator with Boston Children’s Hospital Accountable Care Organization (ACO)'s MassHealth Health Related Social Needs (HRSN) Services Program to address social determinants of health for children, teens and young adults, and their families who are facing nutrition and/or housing insecurity across our four BCH Primary Care sites. The Patient Navigator will be primarily responsible for providing case management support to HRSN enrolled patients and families, which may include but not limited to, conducting initial needs assessment, developing goals and benchmarks, planning appropriate services or interventions, coordination of care and resources, monitoring progress and service delivery, and continued advocacy in navigating social systems.

The Patient Navigator will be responsible for: 
  • The Patient Navigator will be responsible for up-to-date documentation and timely data collection in EHR,  in addition to providing enrollment support to HRSN enrollment team, as needed. This Patient Navigator will work closely with the HRSN centralized team, hospital-wide interdisciplinary teams, primary care sites, ACO staff and community collaterals. 
To qualify:
  • High school diploma/GED required. Bachelor’s Degree in Human Services or a related field is preferred
  • Minimum of one year of closely-related experience required. Experience with the clinic population or work with community housing and/or nutrition resources is preferred
  • Fluency in Haitian Creole, Cape Verdean Creole and/or Spanish strongly preferred. Knowledge of local and state-wide resources and background in social services strongly encouraged. Interest in working with multicultural patients with complex needs, demonstrated cultural humility/responsiveness, strong communication (written and verbal) and interpersonal skills, willingness to work in a fast-paced, team-oriented environment, and ability to problem solve are required. This is a hybrid position. 
Please note: During a public health emergency, individuals in this role may be expected to take on additional duties to respond to organizational needs.
