The Medical Social Worker is a licensed social work professional who is responsible for the delivery of clinical social work services to meet the medically related psychosocial needs of patients and families served by the hospital, in a manner reflective of the department's philosophy and objectives. Provides consultation to hospital and medical personnel related to biopsychosocial issues, continuity of care and discharge planning functions.

I.    Major Responsibilities: 
1.    Provide individual & group therapy to patients with varied mental health/ co-occurring disorders. 
2.    Responsibilities include completing initial assessments, counseling, group therapy, case presentations to the treatment team, treatment planning, and aftercare planning. 
3.    Clinicians are also responsible for identifying the need for medication evaluations and making referrals to the on-site providers. 
4.    Completes all documentation and paperwork specific to the department and in compliance with hospital requirements, DMH, BSAS, The Joint Commission, and insurance providers.
5.    Responsible for managing caseload which may include consultations, collateral contact, and following up with the patient on missed appointments. 
6.    Work towards or maintain highest level of professional licensure and maintain credentialing necessary for specific role

II.    Position Qualifications:
License/Certification/Education: 
Required:
1.    Graduation and training from an accredited graduate (Masters) program such as Social Work, Psychology, Counseling, etc. 
Preferred:
1.    License (LICSW, LMHC, LMFT) eligible.

Experience/Skills:
Required:
1.    Must have strong and effective communication, organization, and time management skills. 
2.    Must be able to work as part of a robust multi-disciplinary clinical team.
Preferred:
1.    Internship and/ or work experience within behavioral health including addiction services. 
2.    Strong diagnostic skills and abilities.
Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.
Department-specific competencies and their measurements will be developed and maintained in the individual departments.  The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents.
III.    Physical Demands and Environmental Conditions:
Work is considered sedentary. Position requires work indoors in a normal office environment.
 

GENERAL DESCRIPTION: Provide quality clinical treatment to clients as assigned by the Intake Coordinator and/or the Department Director.  Provide individual, group, and family therapy to clients of varied ages and stages in recovery.  Complete all documentation and paperwork specific to each department and in compliance with hospital requirements, DMH, BSAS, The Joint Commission, and Insurance Providers. Responsible for managing caseload through consultations, collateral contact, and following up with the client on missed appointments. Maintain Productivity Benchmarks, Maintain current level of professional licensure, & Maintain CAQH credentialing.

WORK EXPERIENCE REQUIREMENTS: 2 years of experience working with children, adolescents, and families in variety of mental health disorders in a clinical setting.

EDUCATION REQUIREMENTS: Graduation and training from an accredited graduate program (MA/MSW) and is licensed eligible.
